About the Position:
The Community Systems Navigator will perform an array of services with the intention of creating pathways for individuals exiting institutions in Los Angeles County who have not been able to get the needed help in terms of Housing, Mental Health, and reliable linkages to community resources in order to help minimize recidivism and establish positive community growth.


What You Will Do:
  • Interview and assess participant needs through interviews with participants referred by the Office of Diversion and Reentry (ODR), Specialized Treatment for optimized programs (STOP), Parole, and other Community Based, Organization Referrals.
  • Novel Pilot partnerships between Amity and Parole offices in engaging eligible students into needed services.
  • Verify and document the eligibility of participants
  • Identify services with the participant to provide proper referrals and linkages within the community.
  • Assist with public benefits advocacy and linkage.
  • Work with other projects, departments and agencies to assist with placement into case management services upon release.
  • Collaborate with other area agencies and partners involved with the participant to ensure a coordinated effort to best serve the participant.
  • Attend inter-agency case coordinated meetings, coalitions, regional advocacy groups, and boards as appropriate in benefiting the participants served and support community cooperation in addressing areas of need.
  • Maintained concise and comprehensive records on all participants seeking services.
  • Provide accurate daily and monthly statistical information as per funding source guidelines.
  • Will be required to visit county jails and parole offices.
  • Provide transportation assistance to housing and appointments by means of company/personal vehicle, bus fare/pass, or private vendor upon approval.
  • Conduct risks/needs assessment and input information into designated system to tract and report out data to management for reporting purposes.

  • A Bachelor's Degree in behavioral health or social science and one year of experience providing outreach, substance abuse counseling, OR the equivalent of a high school diploma and three cumulative years of experience of administrative responsibility in providing outreach, substance abuse counseling, reentry services, and case management within a culturally diverse population.
  • Knowledge on how to administer assessments, substance abuse counseling, reentry services, and case management within a culturally diverse population, preferably with both men and women.
  • Bilingual, fluent Spanish/English oral and written skills are highly desirable.
  • Maintain valid driver’s license and car insurance/registration.
