Position Summary: Case planning with goal of permanency for children either through reunification with parents, through adoption or kinGAP, or in preparation for independent living. To assure a safe and permanent outcome for every child with a focus on safety and timely permanency. 

Responsibilities:  

  • Assess safety and risk as well as protective and promotive factors and collaborate with families to create safety plans. 

  • Continuously identify with the child and family their strengths, resources, and supports needed, and engage families to actively participate in the development and assessment of their service plan. 

  • Work side-by-side with the caregiver/foster parent to support and provide treatment to the child in care.  

  • Collaborate with parent advocates to encourage and support parents in taking steps to address personal and/or family challenges so that their children can safely return home. 

  • Engage families in concurrent planning from the initiation of the child welfare intervention  

  • Identify, support, and develop appropriate family and non-kinship resources to support the child’s goal of permanency  

  • Coordinate with children’s caregivers, schools, medical, mental health and other service providers, and support networks to enhance children’s well-being. 

  • Have ongoing collateral contact with the interdisciplinary team and all service providers to monitor progress and efficacy. 

  • Comply with all Agency, City, State, and Federal requirements related to client contacts, visitation, documentation, mandates, and procedures.  

  • Ensure preparedness, compliance, and participation in all legal matters including paperwork, reporting, and testimony in Family and Criminal Court proceedings. 

  • Complete timely progress notes, FASPS, Psychosocial, Plan Amendments, Adoption summaries, Home studies, court reports, CCS21+ applications, and other required documentation 

  • Schedule, lead, and attend all routine and emergency Family Team Conferences  

  • Develop visiting plans for youth with their with families, viable resources, including separated siblings  

  • Schedule, supervise, and provide coaching of visitation as needed, appropriate, and required. 

  • Prepare individual IL monthly contacts and documentation 

  • Serve as a mentor to less seasoned staff and support onboarding activities for new department members 

  • Crisis Intervention as needed 

  • Assist with hospitalization and discharge planning following hospitalization 

  • Coordinate and facilitate family and sibling visitation/meetings. Supervise as required by the service plan.  

  • Conduct home visits to parents, foster parents and discharge resources 

  • Collaborate with homefinding in identifying, developing, and certifying viable resources and supports for youth 

  • Collaborate with other JCCA divisions to ensure the child’s safe placement in the least restrictive level of care 

  • Complete Special/Exceptional Rate Requests and Reauthorization as necessary 

  • Implement evidence-based models with caregivers, parents, discharge resources and youth. 

  • Attend department and team meetings, consultation and supervision. 

  • Provide extensive outreach advocacy services, and support groups for families as needed 

  • Coordinate with other agency and community based programs such as Health Homes, Behavioral Health and ABC.  

  • Participate in all trainings including training for evidence-based models and other trainings in support of model implementation. 

  • Work with Youth skills coach to develop and teach youth appropriate skills. 

  • Any other duties assigned

Qualifications:  

  • Engage with clients to establish a professional relationship.   

  • MA required, MSW preferred  

  • Knowledge of community resources and ability to effectively negotiate systems for multi-problem families.  

  • Experience with drug treatment populations preferred 

  • Typing skills and computer literacy required 

  • Excellent interpersonal skills 

  • Spanish/English preferred 

  • Drivers’ license
