Lutheran Homes & Health Services, an Illuminus Community, is looking for a Referral Specialist to join their Senior Care community. We have been a part of the local community caring for the families of Fond du Lac and the surrounding area since 1936. Conveniently located in the heart of downtown Fond du Lac, Lutheran Homes & Health Services provides assisted living, skilled nursing and short-term rehab services to older adults.

JOB SUMMARY:
The Referral Specialist is responsible for maintaining census goals (SNF & A/L) while managing all referrals to the campus health centers from; receipt of the referral, approval of payment source, health center readiness for admission, collaboration with referral sources, to; welcoming and on-boarding the newly admitted individual.
ESSENTIAL JOB FUNCTIONS:
- Maintains and actively promotes effective communication with all individuals.
- Maintains a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
- Responsible for on-call services, as assigned.
- Receives and responds to referrals and/or inquiries swiftly with a desire to convert the referral into an admission.
- Reviews and identifies clinical and equipment needs of prospective residents with the interdisciplinary team.
- Communicates effectively with referral sources.
- Coordinate referrals and process admissions.
- Assist in obtaining hospital documentation necessary to complete Minimum Data Sets.
- Create contacts that enhance development of business partnerships
- Performs other duties, as assigned.
Requirements
QUALIFICATIONS:

EXPERIENCE AND TRAINING:
- Health Care Administration, Social Work, or Registered Nurse preferred.
- Understanding of the Acute Care and Post-Acute Care Transition Cycles.
- Familar with SNF and A/L Regualtions and payor sources
