Position Summary
The Care Coordination Nurse will provide care coordination for patients with Hepatitis C infection who are in treatment for opioid use disorder at Greenwich House Center for Healing. The Care Coordination Nurse will work with an interdisciplinary team comprised of on-site Hepatitis C treatment providers, counselors, and a Hepatitis C CRPA in order to educate patients about prevention, treatment, and harm reduction. Additional responsibilities include engaging and supporting patients in treatment for Hepatitis C, tracking patient progress, assisting the medical team with acquiring medications, providing and managing Hepatitis A and B vaccinations, tracking cure rates and reinfections, and coordinating general medical care for patients engaged in Hepatitis C treatment.
Responsibilities:
- Educate patients about Hepatitis C infection
- Manage and oversee implementation of annual Hepatitis C screening of patients
- Facilitate referral to on-site medical providers for treatment of Hepatitis C
- Track progress and cure rates of patients receiving Hepatitis C treatment
- Provide Hepatitis A, Hepatitis B, and influenza vaccinations
- Manage vaccine ordering, tracking and documentation
- Provide injections of injectable medications as ordered by the provider
- Assist in education of staff on Hepatitis C infection and treatment
- Provide harm reduction counseling to patients with the goal of reducing overdose deaths, infectious disease, and Hepatitis C reinfection
- Track population data on Hepatitis C infection rate within clinic patient population
- Review laboratory tests with patient and provide patient education
- Educate patients on HCV transmission, treatment, prevention, and harm reduction in individual and group sessions
- Arrange trainings for staff on wound care and manage wound care supplies
- Develop protocols for medical and nursing staff on management of wounds
- Participate in regular meetings to review program progress, identify patient needs, and review infection and cure rates
- Develop protocols for prior authorizations and delivery of Hepatitis C medication to clinic
- Collaborate with nursing team to provide directly observed Hepatitis C treatment when indicated
- Supervise and work collaboratively with HCV certified recovery peer advocate
- Perform phlebotomy services when needed
- Dispense medication when required to meet patient needs
- Facilitate referrals to specialty and primary care
- Work collaboratively with an interdisciplinary team of medical providers, directors, certified recovery peer advocates, and counselors to ensure maximal access to treatment and engagement of patients in Hepatitis C treatment
- Perform other tasks and special projects as assigned by the direct supervisor
Qualifications:
- Licensed Registered Nurse in New York State, required
- Experience providing care for people with substance use disorders or Hepatitis C, preferred
- Skilled in establishing rapport with patients who have substance use disorders and other mental health conditions
- Embraces harm reduction as essential in treatment for Hepatitis C and opioid-use disorder
- Skilled in communication, organization, leadership, and clinical care
- Works well in interdisciplinary teams
