Summary of Job Description:
Incumbent serves as a liaison for Fort Hill and community by providing case management to ensure both residents and community members receive services to support their healthcare and other needs (including those requiring assisted living) in coordination with both internal and external service agencies. Incumbent also serves as part of an interdisciplinary team for planning, implementing, coordinating, monitoring, and evaluating residents’ needs and how best to meet them.
Minimum Requirements:
- Critical thinking skills and ability to anticipate and prioritize residents’ needs
- Ability to ascertain an individual’s participation in activities based on cognitive capacity and physical abilities, interests and needs
- Strong verbal, written and interpersonal skills; ability to interact effectively with residents, their families, and internal and external agencies and staff
- Demonstrated knowledge of Medicare, medical insurance, long-term care insurance and coordination of benefits
- Ability to understand and work within established budgets
- Knowledge of and ability to communicate medical terminology
- Ability to understand and communicate knowledge of population-specific prescribed medication and familiarity with common disease progression and treatment plans
- Ability to react calmly and effectively in emergency situations and to manage difficult resident behaviors
- Ability to work independently and as part of an interdisciplinary, collaborative team
- Basic word processing, spreadsheet, and data entry skills

- Bachelor’s Degree in Social Work, Human Services or similar field and one (1) year of experience in a social service delivery system
~OR~
- Associate’s Degree in Social Work, Human Services or similar field and three (3) years of experience in a social service delivery system
~OR~
- High school diploma or equivalent and five (5) years of experience in a social service delivery system
~AND~
- Experience in case management and coordinating services for the disabled or those primarily aged 55 or over
- Prior experience working with external service agencies in the community
- CPR /AED (Automated External Defibrillator) certified or the ability to be certified within one (1) year of employment
- Valid motor vehicle operator license
- CDL Class B with P endorsement is required or Class D with F endorsement required

- Successful candidate must pass the background check and continue to be eligible for employment in accordance with The Mohegan Tribe’s Indian Child Protection and Family Violence Prevention Policy (Policy #80)
- No felony convictions or misdemeanors involving moral turpitude or issues of integrity
