SPECIFIC DUTIES AND FUNCTIONS:
- Supervises and coordinates Home Care Activities at PACE Southeast Michigan (PACE SEMI),
 - In collaboration with the Center Manager and IDT, facilitates the Participant-Centered Planning meetings, as well as complex Care Plan Development/Enrollment meetings, to assure consistency in care planning and service authorization for participants enrolled at PACE SEMI.
 - Assists the Medical Director as requested in duties related to clinical and operational issues and special projects as may be required.
 - Provides home care authorizations for external vendors as needed based on staffing to ensure participant needs are met
 - Teach, coach and mentor schedulers along with Direct Care staff.
 - Communicates with the Director of Home Care regarding case management/homecare operations and issues.
 - Facilitates administrative, quality and performance improvement requirements, such as reports and statistical records.
 - Provides weekly report to schedulers regarding staff productivity.
 - Participates in and/or facilitates Quality Assurance projects resulting from data results.
 - Ensures adherence to departmental and external standards in the provision of quality focused care by attendance at professional meetings/committees and review of national standards of practice.
 - Ensures adherence to budgetary guidelines.
 - Other duties as assigned.
 - Interview, hire, onboard staff.
 
GENERAL REQUIREMENTS:
- Bachelor required, Bachelor of Science in Nursing preferred.
 - Preferred three years of clinical long-term care experience, including one year with frail elderly population, and two years of supervisory experience.
 - Has ability to lead and direct other licensed staff in the delivery of care. Only acts within scope of his/her practice.
 - Must meet a standardized set of competencies (approval by CMS) before working independently.
 - Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
 - Must meet State Licensure requirements (be legally authorized to practice in Michigan, and only act within the scope of his/her authority to practice).
 - Excellent oral and written communication skills necessary; must have strong interpersonal skills.
