About Us
High Point & Affiliated Organizations is a health and human service agency whose mission is to treat and prevent substance use disorders and mental illness. High Point has programs located throughout Southeastern Massachusetts offering a full continuum of care for substance use and mental health treatment, including inpatient, outpatient, residential, and community-based services. Nurse Case Manager Requirements
Must present as courteous and professional at all times.
Demonstrated flexibility and ability to perform multiple tasks.

TB screening
Certification in the HPAO approved de-escalation program
Strong and effective verbal, written and organizational skills
Knowledge of basic computer skills and software programs including the ability to learn, use and train HPAO`s electronic medical records
Ability to provide leadership and program development within an outpatient site
Knowledge of major clinical therapeutic models and behavioral health best practice
Strong organizational, verbal and written communication skills.
Nurse Case Manager Duties & Responsibilities
Compliance with the Federal Regulations, 42 CFR Part 2, Confidentiality of Alcohol and Drug Abuse Patients and HIPAA Regulations;
Compliance with DPH, DMH, Joint Commission, DEA and all other regulatory agencies as required;
Knowledge of ASAM criteria;
Monitoring of patient needs related to filling practitioner orders, administration of medications, monitoring vital signs, coordinating medical care, monitoring withdrawal (COWS/CIWA);
Assistance with medication prior authorizations;
Assist with laboratory (blood) draws, supply monitoring, and sample handling;
Assist with urine drug screening, ordering of labs, and results reporting to practitioner;
Complete case coordination activities to ensure medical care coordination, including: PCP contact within 72 hours of new intakes and every 6 months, obtaining last physical report (as available), contact with patient within 72 hours post-hospitalization, contact with patient 24 hours post-ER visit when notified, medication reconciliation upon intake, and other care coordination activities as assigned by Program Director/Clinical team;
Assist with discharge planning and medical care coordination activities;
Provide brief crisis counseling, individualized risk management and safety planning;
Provide psychoeducation to both patients and staff related to medical/nursing areas of expertise;
Ensure patient care is meeting best practice standards;
Ensure compliance with all governing regulations and payor guidelines;
Ensure compliance with High Point policies and procedures;
Participate in the recruitment, hiring, promotion, training, and evaluation of nursing staff;
Assist in developing nursing and medication records on each patient;
Reviews and makes recommendations regarding policies and procedures;
Reports to Infection Control Committee as needed;
Assist in monitoring inventory, ordering of medications, and equipment needed for daily operation of the unit in collaboration with the pharmacy;
Responsible for the fiscal management i.e. nursing budge for the unit;
Participate in quality assurance and implementing quality improvement planning, including Tally Card, CQI and/or failure mode projects;
Reviews Incident Reports and Sentinel Events, as required;
Provide supervision for nursing staff, medical assistant/CNA personnel;
Participate in all required committee meetings;
Responsible for the proper disposal of medications;
Responsible for the scheduling of nurses in order to maintains adequate nursing staffing patterns in order to manage the day-to-day operation of the unit;
Monitors both open and closed charts for correct and complete documentation;
Assists in the development and monitoring of programming schedules;
Attends monthly Nurse Managers meeting to maintain consistency in nursing practice;
Attend mandatory training to update knowledge base and enhance both professional and personal growth;
Other duties as assigned by the Director of Nursing, Site Director of CBHC Services/ COO to insure safe and appropriate operation of the program(s).
Nurse Case Manager Qualifications
- Five to eight years of direct care experience is recommended within a behavioral health &/or substance use disorder treatment setting
- Two years supervisory experience preferred
- Current & unrestricted license in Massachusetts as a Registered Nurse
