PACE TRANSPORTATION COORDINATOR/DRIVER
GRADE: 5 Non-Exempt REPORTS TO: PACE Transportation Supervisor
POSITION REQUIREMENTS: Associate degree or high school diploma with equivalent experience may be considered; minimum two (2) year's experience in the area of transportation, human services, disabilities, or other related field is required; advanced knowledge of computers is recommended; must have demonstrated excellent interpersonal communication skills. Must have a valid driver license and pass a criminal background check.
SUMMARY: The Program for All-inclusive Care of the Elderly (PACE) Transportation Coordinator/Driver on an as needed basis will assist the PACE Transportation Supervisor with the following duties: work as a member of the PACE Interdisciplinary Team (IDT) to assess participant transportation needs and make recommendations to the IDT; will determine appropriate mode and provision of transportation services; will serve as the PACE transportation liaison with Four County Transit and a local network of providers; must maintain effective communication with providers, drivers, and the medical community; must work independently and use sound judgment in coordinating appropriate transportation services; will assure transportation services are provided in accordance with federal, state, and local guidelines.
DUTIES AND RESPONSIBILITIES:
- Will assist the PACE Transportation Supervisor in receiving referrals for potential PACE transportation services.
- Assist the Supervisor with schedules and conducting an in-home assessment on each potential PACE participant to determine individual needs, evaluate barriers to service delivery, and to coordinate appropriate transportation services to and from medical appointments and social activities, such as adult day health care.
- When the Transportation Supervisor is not available the Coordinator will serve as a required member of the PACE Interdisciplinary Team, presenting findings of the in-home assessment along with a plan to deliver the appropriate transportation services upon participant enrollment.
- The Coordinator on an as needed basis will attend morning meetings to report on any occurrences relative to transportation and the PACE participant.
- Will assist in pre-authorizing all PACE transportation services in accordance with PACE guidelines and the participant plan of care.
- The Coordinator will assist with training of PACE Drivers and problem solve as issues arise.
- The Coordinator will assist in developing and maintaining a local transportation provider network, including PACE Drivers, ambulance (emergency and non-emergency), stretcher van, wheelchair van, and taxi services to deliver transportation services as needed.
- Working as the PACE liaison, the Coordinator will schedule transportation through Four County Transit, or the local provider network, on a standing schedule (ADHC, dialysis, etc.), demand response basis, or on an emergency or non-emergency basis.
- The Coordinator will schedule PACE Drivers, coordinate, and/or dispatch transportation services in an appropriate and timely fashion.
- As follow-up to delivery of service, the Coordinator will communicate with the service provider (s), including drivers, to determine satisfactory completion of trips, address any problems which may have occurred during transport, participant behavior, etc.
- The Coordinator will work with PACE Drivers, Four County Transit, and the local provider network, to coordinate the most cost effective and efficient transport of PACE participants, i.e. multiple pick-ups; coordinated appointments, etc.
- The Coordinator will establish and maintain a good working relationship with the PACE participant, their caregiver (s), medical community, including hospital emergency rooms, dialysis centers, physician offices, and other providers.
- The Coordinator will conduct ongoing evaluations of existing transportation provider services to assure adequate capacity, identify any service limitations (i.e. lift equipped vehicles, taxi service, escorts, bariatric services, etc.) and make recommendations to the appropriate provider staff.
- The Coordinator will help with ensuring maintenance of PACE vehicles.
- The Coordinator will utilize problem solving skills to deal with non-routine requests for transportation services, i.e. night/weekend/holiday calls, out-of-state, overnight transports, med-flight, etc.
- Perform duties of PACE Driver when necessary.
- The Coordinator will assist in generating all PACE program reports as required.
- The Coordinator will comply with HIPAA requirements and will maintain strict confidentiality in all matters pertaining to PACE participants and program operations.
- The Coordinator will attend training as required.
- The Coordinator will perform other appropriate duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
- Must have a working knowledge of transportation services, including medical transport
- Ability to independently assess client transportation needs
- Ability to use sound judgment in developing a plan for service delivery
- Ability to make decisions in routine and non-routine situations
- Ability to work as a member of an interdisciplinary team (IDT)
- A working knowledge of the PACE service area
