Position Title: Case Manager – Supported Accommodation
Location: Toowoomba, QLD
Employment Type: Full-time, Permanent
Award Classification: SCHADS Award – Crisis Level 2
Work Hours: 38 hours per week, Monday to Friday (between 8:00 AM – 8:00 PM; mobile outreach support)
On-Call: Participation in a rotating on-call roster is required

About the role:
We are seeking a passionate and skilled Case Manager to join our Toowoomba Supported Accommodation team. This role provides critical support to individuals in both residential and community settings, helping them work toward housing stability, social connection, and long-term independence.

How You Will Make an Impact:
As a Case Manager, you’ll play a pivotal role in delivering services aligned with The Salvation Army’s National Homelessness Stream Model of Care. You’ll provide client-centered, strengths-based case management tailored to the unique needs of each individual.
Key Responsibilities:
- Build positive, respectful relationships with clients, encouraging empowerment and engagement
- Conduct comprehensive assessments of client needs, strengths, risks, and vulnerabilities
- Develop collaborative and personalised case plans with clearly defined goals and pathways
- Implement and regularly review case plans to ensure they remain responsive and effective
- Provide ongoing support through every stage of the client’s journey—from intake to exit
- Deliver services that align with best practice models and The Salvation Army’s mission
- Liaise with internal and external services to enhance client outcomes and referral pathways
What You Will Bring:
- Tertiary qualification in Social Work, Welfare, Community Services, or a related field (degree preferred)
- OR significant sector experience supported by professional development and demonstrated capability
- Proven experience in case management, client assessment, and service delivery within the social services sector
- Current QLD Working with Children Check (Blue Card)
- First Aid Certificate (if designated as a first aider)
- Current and valid QLD Driver’s Licence
