Description

Hospice of the Piedmont is seeking a compassionate and experienced Palliative Nurse Practitioner to join our dedicated team. As a mission-driven organization, we are committed to making a life-changing impact by delivering expert care to individuals in our community facing serious illness. 


In this role, the Nurse Practitioner will provide direct care and clinical expertise to individuals, families, and groups in home or facility settings. Working closely with Medical Directors, NP colleagues, and interdisciplinary staff, the NP is responsible for the assessment, planning, implementation, and evaluation of care for current palliative patients—ensuring services are delivered with the highest level of skill, compassion, and respect.


This position primarily serves Augusta, Nelson, Buckingham, and Fluvanna counties. We welcome candidates interested in working between 30 to 40 hours per week. 


Responsibilities/General Duties (Including but not limited to):

Direct Patient Care

- Provides palliative care visits and coordination of care in various settings (home, ALFs, SNFs, clinics).

- Assists with diagnosis, treatment, and management of acute and chronic health conditions.

- Interprets, evaluates, and communicates diagnostic findings.

- Prescribes medications, including controlled substances, in collaboration with Medical Directors and PCPs.

- Participates in rotating on-call responsibilities to ensure 24/7 medical coverage.

Care Coordination & Collaboration

- Collaborates with referral sources and PCPs to ensure quality services.

- Consults with referring physicians, the palliative team, and other providers to contribute to the patient’s plan of care.

- Facilitates and documents clarification of patient and family goals of care.

- Maintains effective communication with coworkers, IDG, patients, and families.

Education & Outreach

- Assists in outreach activities to promote and educate about palliative services.

- Participates in community education through student/resident/fellow rotations, lectures, research, and presentations.

- Educates co-workers, facility staff, and the community on palliative care.

Documentation & Quality Standards

- Prepares and maintains accurate patient records, charts, and documents to support medical practice and reimbursement.

- Adheres to confidentiality practices regarding patients, families, staff, and the organization.

- Assists in the development of clinical practice guidelines/standards to support quality care.


Qualifications:

- Licensed to practice in the Commonwealth of Virginia

- Certified as a Clinical Nurse Specialist or Nurse Practitioner

- Certification by a national body (ANCC, NBCHPN, AANP)

- Palliative care ACHNP preferred

- Minimum of five years nursing experience; clinical nursing care in geriatrics, long-term care and/or palliative

- Driver’s license, personal automobile and current automobile liability insurance


Typical Physical Demands/Working Conditions

- Ability to sit, stand, stoop, bend, reach, lift, walk, and climb steps

- Verbal and written communication skills, including ability to use phone and computer efficiently

- Requires normal visual acuity and hearing

- Drive short/long distances

- Frequent exposure to communicable diseases, bodily fluids, toxic substances, medicinal preparations and other conditions common to a clinical environment
