POSITION SUMMARY: 

 Under the direction of the Site Manager of Adult day services and the Office Manager, this position will be responsible, for driving a minibus, van, or lightweight vehicle to transport clients or participants with disabilities. This role will play a dual role in this department as will be a part-time Activity Aide, this function will mainly provide support in the care and administrating of daily cognitive and stimulating activities for the clients in the Adult Day Services department.

Chicago Commons provides services that help seniors and adults with disabilities maintain their independence and quality of life while remaining in their homes. Our Adult Day Service (ADS) program provides a professional care setting in which seniors and adults with disabilities are transported to our center to receive individualized care in a community-based group setting. Our Adult Day Service center is located in Bronzeville and provides daily onsite care for seniors and adults with disabilities.

ESSENTIAL DUTIES & RESPONSIBILITIES:

These include but are not limited to the following.  (The company reserves the right to add to, delete, change or modify the essential duties and responsibilities at any time). 

Arrange for vehicle repair with the assistance of the ADS Manager. Follow established schedules; adjusts schedules to meet unanticipated changes; determine the safest and most efficient pick-up and delivery routes and other destinations according to schedule.  

Responsibilities:

  • Assist passengers on and off the vehicle in a safe and courteous manner, assist clients that use medical equipment, and handle emergencies that may arise.
  • Follow daily schedule assigned to transport clients to numerous areas within the city of Chicago.
  • Provide a high level of customer service to clients enrolled in the program.
  • Note visible changes in clients' physical or mental health regularly transported; communicate this information to appropriate personnel for follow-up. Document unusual behavior.
  • Secures wheelchairs to restraining devices to stabilize wheelchairs during transport.
  • Ensure that clients are received by the caregiver and authorized person.
  • Conduct weekly vehicle inspections. Thoroughly inspect the vehicle and maintain a clean and safe condition. 
  • Report the need for maintenance and/or service to the ADS Manager.
  • Maintain standard records such as logs of passengers, mileage, and gasoline purchases.
  • Operates radio or similar device to communicate with the base station, other drivers, and caregivers or to report disruption of service.  
  • Program Aide Assistants help provide personal care such as toileting, bathing, eating, transfers, and grooming to Adult Day Service clients.  
  • Assist bus drivers with loading and unloading clients.  
  • Assist clients with removing coats upon arrival and properly identifying coats (with tags), hats, gloves, and other personal items before hanging them up. 
  • Assist the lead staff in caring out activity programs.  
  • Gathers prepares and maintains all equipment, materials, and supplies for activities. Keep the program area organized and clean. Assist with the serving of breakfast, lunch, and snacks. 
  • Assist with kitchen duties and menu planning. Assist with food inventory control and food preparation.  
  • Assist with food temperatures upon delivery from the caterer.  
  • Assist with observing clients and report activities to appropriate staff.  
  •  Complete client forms and document information in files. Participate in team meetings and in-service educational training.  
  • Other job duties are assigned.  

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below represent the knowledge, skill, and/or ability required.   

Valid Illinois Drivers License.
5 years of experience working with seniors or adults with disabilities.
5-years years of driving experience.

EDUCATION: 

  • High School diploma/GED.
  • CPR certification is required.

WORKING CONDITIONS:

There are no unusual environmental conditions other than inclement weather and inappropriate driving of other vehicles. 

PHYSICAL REQUIREMENTS: 

  • While performing the duties of this job, the employee is regularly required to speak English, specifically, to communicate both in person and over the telephone.
  • The employee must be able to read English proficiently.
  • Must be able to assist with the transfer of ADS participants. This could require lifting.
  • The employee frequently is required to sit and use hands and fingers to handle, or feel objects, tools, or controls. The employee is occasionally required to stand; walk; reach with hands and arms, and stoop, kneel, crouch, or crawl.  
  • The employee must occasionally lift and/or move up to 50 pounds. Specific vision requirements include the ability to see at close range.
