💼 Occupational Therapist (O.T.) – Job Summary

As a Home Health Occupational Therapist, you will provide in-home therapeutic services to patients recovering from illness, injury, or surgery. You’ll assess patient needs, create personalized therapy plans, and work collaboratively with an interdisciplinary care team to deliver high-quality care in a home setting.

🛠️ Key Responsibilities

• Evaluate patients’ functional abilities and develop individualized occupational therapy plans.

• Deliver hands-on, in-home therapy focused on daily living skills, motor function, and independence.

• Monitor patient progress and modify treatment plans accordingly.

• Collaborate with nurses, physical therapists, physicians, and caregivers to ensure coordinated care.

• Maintain accurate and timely clinical documentation in accordance with agency and state standards.

• Educate patients and families on therapy goals, safety techniques, and use of adaptive equipment.

✅ Qualifications

• Graduate of an accredited Occupational Therapy program.

• Current Occupational Therapist license in the state of California.

• Registered with the National Board for Certification in Occupational Therapy (NBCOT).

• Minimum 1 year of experience as an Occupational Therapist (home health experience is a plus).

• Reliable transportation and a valid driver’s license.

• Strong communication and time-management skills.
