The Nursing Clinical Care Coordinator is primarily responsible for managing admissions, transfers, discharges, and patient movement, as well as clinical/administrative follow-up on specific patients as assigned by the Associate Director, Inpatient Unit(s). In addition, the Nursing Clinical Care Coordinator provides, when needed, direct care services throughout the hospital. This includes inpatient, admissions, residential care and interventional psychiatry. This position reports to the Associate Director of Nursing. 

Schedule:

  • Monday through Friday working from 8:00am to 4:30pm, hours subject to change based on departmental needs.
  • Standard 40-hour work week. 

Duties/Responsibilities:

  • Manage admissions, discharges, transfers through Doctors Rounds providing guidance on balancing the needs of the patient, unit, and staff.
    • Monitor Dashboard for quality, safety, and infection control metrics that may indicate a challenge or an opportunity for improvement.
    • Evaluate the effectiveness of multidisciplinary treatment planning, supporting nursing contributions.
    • Coordinate nursing care as required.
    • Develop collaborative relationships with the physicians to manage patient logistics.
  • Provide clinical follow-up on select patients as assigned by the Associate Director Nursing Inpatient Unit(s).
  • Provide support for quality initiatives/projects, as needed.
    • Support/manage specific data collection activities.
    • Participate in specific quality initiatives/projects as assigned by the Director, Practice Excellence. 
  • Provide educational experiences to support the professional growth of nursing staff by providing high quality, safe, evidence-based, culturally sensitive, compassionate care. 
    • Deliver orientation classes when needed.
    • Deliver other clinical classes when needed.
  • As needed, provide direct care RN services throughout the hospital. This includes inpatient, admissions, residential care, and interventional psychiatry. 
  • Participate in Clinical Rounds (on the units with nursing staff) with a specific purpose to raise the level of professional practice and build a culture of expectation of excellence, autonomy, and accountability. 
  • Performs other duties and responsibilities as assigned.

Required Skills/Abilities: 

  • Must have the ability to function optimally in a stressful environment, and the ability to remain calm in emotionally charged situations.
  • Able to speak and write fluently in the English language.

Education and Experience:

  • Bachelor’s degree in Nursing preferred.
  • Licensure as a Registered Nurse in Connecticut.
  • Certification in Substance abuse disorders or Psychiatric Mental Health Nursing preferred.
  • CPR BLS certification, after hire and before assignment to direct patient care.
  • Experience with psychiatric and substance abuse disorder patient populations.
  • At least 3 years experience as a psychiatric nurse.
  • Experience in teaching, staff development, quality preferred.
  • Prior supervisory experience preferred.
