Role: Home Health RN Case Manager

• Location/Type: STL Metro Suburbs • Field/Hybrid
• Pay: $83,000–$96,200 salary + mileage
• Schedule: Mon–Fri, no weekends, no call

What you’ll do

• Manage 4–6 patient visits/day (not overloaded)

• Complete OASIS start-of-care, recerts, discharges

• Coordinate with PT/OT/SW (same-day response)

• Teach patients/families care plans at home

• Document in EMR (built for speed)

• Report patient changes quickly to clinical lead

Must-haves

• Active RN license in MO

• 1+ year home health RN experience

• 1+ year OASIS (SOC/ROC/Recert/Discharge)

• Valid driver’s license + reliable car

• Able to cover Clayton, Chesterfield, Creve Coeur, Kirkwood, Ballwin, Florissant, Webster Groves, St. Charles County

Nice to have

• Broader suburban reach into Jefferson County (Arnold, Imperial)
