BronxWorks is seeking one Program Coordinator/Quality Assurance Specialist/Continuous Quality
Improvement Specialist to join a new, diverse and enthusiastic team at Bronx Point, a new supportive housing program located in the Bronx, NY. Bronx Point provides services and permanent housing to 82 formerly homeless households, consisting of both individuals and families with children. The ideal candidate is passionate about teamwork, improving the quality of services provided by the program, and will create innovative ways to increase program metrics.
RESPONSIBILITIES
- Collect and report statistical information from AWARDS, TMS and other required databases to write and complete reports.
- Extract data from data sources to identify Important trends, problems or patterns
- Prepare monthly reports on performance requirements
- Present data findings to inter-agency and external agency meetings and funding source
- Submit written reports and analysis that accompany data reporting and presentations
- Document progress notes for all encounters and services provided to clients
- Participate in regular meetings and trainings with supervisor and other support staff
- Train new staff on database usage and assist in onboarding process
- Perform additional duties as assigned
PROGRAM SPECIFIC RESPONSIBILITIES
- Create and maintain logs and sign in sheets for all pans of the Representative Payee service, Metro Card services and use of Petty Cash
- Manage and assist in directing all walk-in clients.
- Maintain and ensure accuracy of records and documentation of all client interactions
- Collaborate with Continuous Quality Improvement (CQI) staff, case management and clinical staff to support program needs
- Attend CQI meetings with other agency employees
- Write initial policies for Bronx Point program based on DOHMH and Bronxworks' standards, rules and regulations
- Implement policies and protocols to Bronx Point to continuously be in compliance with all requirements
- Facilitate trainings on new policies or standards when appropriate
- Assist with preparing for meetings, prepping agendas and training materials
- Maintain expert level knowledge and familiarity with DOHMH funding requirements and standards
- Complete monthly on-site chart audits and communicate results to supervisory team
- Assist in proposal or grant needed as needed for program and department
- Administer the Client satisfaction survey annually and provide survey results to Program Director
- Maintain a list of all client grievances.
- Request rent ledgers from property management to create a rent report for program staff, documenting who is/is not current with rent
- Maintain a tracking record of all program incidents and ensure that Incident Reporting Policy is up to date and in compliance with DOHMH requirements
- Become SOAR Certified in a timely manner.
- Ensure all staff are up to date with the required trainings through record keeping
- Complete Medication Monitoring Training and become certified to assist with audits
QUALIFICATIONS
- Bachelor's Degree
- Proficiency in modern business communications including personal computers, electronic mail, voicemail, cloud-based software (Office 365), copier equipment, virtual meeting platforms (Teams, Zoom, WebEx etc.)
- One year of relevant program assessment and program evaluation experience in professional setting
- Strong oral and written communication, time management and organizational skills are necessary
SKILLS/ABILITIES
- Strong writing and oral communication skills. Strong presentation skills and ability to represent the agency at functions and meetings
- Ability to collect and analyze client demographics and outcome
- Excellent organizational and teambuilding skills
- Ability to use a computer for prolonged periods
- Ability to occasionally lift and/or move up to 10 pounds. Ability to stand, walk, or sit for long periods of time
- Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork
- Ability to bend and retrieve objects and/or documents
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work
