Position Description

The MDS Coordinator (Registered Nurse) coordinates and participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the Care Area Assessments (CAA’s) as indicated. Participates in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes as required by the Federal and State guidelines. Also, serves as the Medical Records Designee.

Job Relationships

  1. Reports to the Director of Nursing.

MDS (Minimum Data Set) Coordinator Duties and Responsibilities

  1. Coordinates the completion of the resident assessment: MDS, CAA’s, in accordance with the current rules, regulations and guidelines that govern the resident assessment.
  2. Maintains the schedule of assessments by keeping items current in the PCC, and communicating changes to the schedule with the center IDT during the daily stand up meeting.
  3. Participates in the family care plan meetings.
  4. Assists with developing resident(s) plan of care in conjunction with other IDT members.
  5. Identifies changes in condition in conjunction with the IDT and coordinates scheduling and completion of indicated assessments.
  6. Collaborates with IDT on provision of appropriate health care services, length of stay for temp. care residents, and discharge planning for these residents.
  7. Participates with resident, family, and team members to make recommendations that maximize treatment outcomes and deliver services in a cost effective manner.
  8. Participates in the facility performance improvement meeting and addresses pertinent items specific to the RAI process.
  9. Attends facility in-services and department/facility meetings as applicable.
  10. Maintains safe and clean work area and adheres to location/company safety standards for the healthcare professional.
  11. Complies with all Infection Control, Standard Precautions and OSHA standards for the healthcare professional as applicable to the position.
  12. Adheres to department/company policies and procedures.
  13. Adheres to established HIPAA confidentiality standards of patient/resident and client location information.
  14. Maintains on-site regular attendance as scheduled.
  15. Completes weekly audits on medical charts as scheduled.
  16. Works closely with Medical Records Coordinator to ensure that all aspects of the chart thinning, breakdown of charts and chart audits have been completed in a timely manner.
  17. Works with the Medical Records Coordinator on a quarterly basis and makes sure that all policies and procedures concerning medical records are compliant with state and federal guidelines.
  18. Gathers data and reports for Risk Management, Quality Assurance and Benchmarking.
  19. Will be responsible for participating in the on-call rotations with the nurse managers.
  20. Assists ADON with COVID testing/reporting as needed
  21. Completes nursing restorative aid charting and reports any concerns related to restorative aid therapy to DON.
  22. Performs all other tasks or duties as assigned.

Typical Physical Demands

  1. Ability to walk up to 20 minutes at a time in order to transverse campus.
  2. Considerable hearing ability in order to perceive the nature of sounds and medical equipment.
  3. Frequent walking, bending, stretching, lifting, pushing and carrying.
  4. Able to see and read “newsprint” sized letters in order to make visual assessments of residents.
  5. Possible exposure to: airborne pathogens, blood borne pathogens, odors, toxic chemicals and fumes.
  6. Ability to lift and maneuver items weighing up to 30 lbs. is required.
  7. Must be able to successfully complete the WorkSTEPS Functional Testing program. This program assists in determining whether applicants/employees are able to safely perform essential job functions.

  1. Must show ability to read and write in a legible and understandable manner.
  2. Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care.
  3. Must display a positive attitude toward the elderly.
  4. Must be a strong, team player.
  5. Computer skills-Windows, Excel (preferred).
  6. Strong verbal communication skills. The ability to read, write, understand, and speak fluently and correctly in the English language.
  7. Ability to embody and uphold the corporate personality and culture of Arbor Acres with its emphasis on warmth, caring, generation of community, enthusiasm, achievement of excellence, and joy.
  8. Bilingual a plus, but not required

Minimum Education

  1. Graduate of an approved Nursing school.
  2. Registered Nurse (RN)

Minimum Experience

  1. Experience in a hospital, long-term facility, or other healthcare related facility.
  2. Experience as an MDS Coordinator is preferred.
  3. Experience in completing MDS’s and Care Plans is required.

Certification/Licensure

  1. Licensed, in good standing, to practice nursing as an Registered Nurse (RN) in the state of North Carolina.
