Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement.

We are currently seeking a Nurse Practitioner (NP) who will perform face-to-face visits with hospice patients to determine their ongoing eligibility for hospice care in alignment with CMS regulations.
Schedule:
Monday - Friday.; 8:30am-5:00pm.
Location:
San Mateo County
**Bi-lingual in Spanish, Cantonese, Mandarin preferred**
Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.
Essential Duties & Responsibilities include (but are not limited to):
- Conduct face-to-face visit with hospice patients to gather clinical findings to determine continued eligibility for hospice care
- Document face-to-face encounter with the patient, including the date of that visit
- Maintain, run, and distribute applicable reports and patient lists
- Synthesize reports to ensure timely completion of face-to-face visits in compliance with CMS regulations
Education/Experience:
- Bachelor’s degree and graduate of accredited Nurse Practitioner Program or Master’s in clinical field of nursing or clinical field related to nursing
- Experience in an acute care, home health, palliative or hospice setting preferred
Certificates, Licenses, Registrations:
- Advanced practice Registered Nurse with active Nurse Practitioner license
- Public Health Nursing certification is highly desirable
- HPNA certification preferred
Pay Range:
$60.00 - $75.00
