PRIMARY FUNCTIONS:
The Cancer Screening Program Coordinator is an integral member of Neighborhood Health’s cancer screening team. They ensure that patients who are referred for breast and cervical cancer screening via the Virginia Department of Health (VDH) - Every Woman’s Life (EWL) project are connected to the cancer screening and diagnostic services that they need in a timely fashion and that patient data is accurately collected, entered, tracked, and analyzed for reporting to funding agencies. This role involves both program oversight and direct patient service, data entry, and analysis. The Cancer Screening Program Coordinator will also be responsible for inputting patient/client data into the VDH-EWL Program database (Catalyst), working in Neighborhood Health’s electronic medical record (eClinicalWorks), and working with the Cancer Screening Program Manager to ensure accuracy of program data and that program metrics are met.
DUTIES/ RESPONSIBILITIES: (Essential Functions: Duties and responsibilities noted with bullets are the essential functions of the job).
- Ensure high quality patient care: Work closely with Neighborhood Health’s cancer screening multi-disciplinary team – including medical providers, nurses, medical assistants, other patient navigators, and the Quality Team – to meet breast and cervical cancer screening goals.
- Participate in monthly cancer screening team meetings to collaborate on strategies to achieve cancer screening goals.
- Assess patient program eligibility: Review all referred patients for EWL program eligibility. Assess reasons for patients declining program participation.
- Schedule mammogram appointments: Work one-on-one with patients to schedule mammogram appointments with local radiology partners; assess patient lifetime risk for breast cancer using standardized screening tools; submit billing forms; send text/voice appointment reminders; and arrange for transportation if needed.
- Ensure completion of breast and cervical cancer screening: Track appointment attendance and receipt of reports. Follow up with patients for whom additional testing is recommended. Conduct appropriate follow-up with patient if screening is not completed. Track reasons for no-shows and identify barriers to screening completion.
- Assess patient cervical cancer screening status and schedule patient for any necessary cervical cancer screening.
- Chart all patient interactions in electronic medical record.
- Ensure all patient data is accurately tracked at all stages of cancer screening process.
- Respond to inquiries about patient data from VDH and other partners.
- Receive incoming referrals: Assist referred patients with making necessary appointments, track patient data, and provide results back to referring providers.
- Assess barriers to timely screening in all referred patients and suggest program improvements to reduce barriers to care.
- Assist with reporting on transportation provided to patients and track grant expenditures.
- Mobile mammography partnerships: Coordinate logistics with partners, office managers, and clinical staff. Assist with scheduling appointments, ensure required patient information is available/accessible for mobile mammography partner, and provide onsite assistance during screening events.
REQUIRED QUALIFICATIONS
Skills, Competencies, Education
- Bachelor’s Degree required
- Bilingual skills required (Spanish/English)
- Previous experience working with vulnerable communities, particularly the uninsured
- Experience with data management: creating, using and maintaining detailed Excel spreadsheets
- Experience working in an EMR or database preferred (training provided)
- Strong organizational skills, attention to detail and accuracy
- Good interpersonal communication and customer service skills
- Knowledge of HIPAA medical privacy requirements (training provided)
- Familiarity with medical terms and technology (not required, but desired)
