- Accountable for overall treatment planning efforts, care conferences with families and providers, family and provider care updates, psychosocial needs and completion.
- Coordinate and facilitate the development of a discharge plan of care for the patient population which includes accountability for the complex discharge process.
- Ensure the experience of care for the patient and family is exceptional.
Experience: Minimum of 2 years of clinical experience in a behavioral healthcare or acute psychiatric setting preferred.
Licensure: Current Social Worker license in the state of AZ is highly preferred
CPR and CPI certification obtained during orientation.#INDEEDMED
