Job Summary:

The Housing Care Navigator is responsible for supporting individuals in the shelter system through ongoing needs assessment, housing plans, referrals, and advocacy for individuals to help them achieve housing stability.The role requires creating individualized service plans, coordinating with external agencies, and using trauma-informed care and harm reduction strategies. This position involves managing a caseload, keeping clear and accurate files, and providing crisis intervention and housing support services.

 

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

  • Conduct intakes and assessments upon arrival, identifying individual needs and barriers to long-term success.
  • Develop individualized housing plans in collaboration with members.
  • Provide referrals and coordinate services with external providers, landlords, and community partners.
  • Deliver trauma-informed, harm reduction-based support and crisis intervention.
  • Manage a caseload, maintaining clear, accurate, and timely documentation in the Homeless Information Management System (HMIS).
  • Advocate for member housing rights, including guidance on lease agreements, tenant responsibilities, and navigating community resources.
  • Facilitate workshops, peer support groups, and orientation sessions as needed.

  • Assist participants in securing identification and documentation necessary for housing access.
  • Support participants in co-developing a personalized housing plan, tailored to their unique circumstances and goals.
  • Provide a thorough orientation on program rules, expectations, and available services.
  • Connect participants with essential resources, including financial assistance, employment support, mental and physical health providers and social services.
  • Meet regularly and consistently with each member to address any concerns or barriers that may impact their housing goal.
  • Transport and /or accompany participants as needed to support with housing search or to make connections with other resources.
  • Provide guidance on lease agreements, tenant rights, and responsibilities to ensure long-term housing stability.
  • Engage with landlords, property managers, and community organizations to expand housing opportunities.
  • Support positive experiences by teaching, being inclusive, answering questions, and supporting residents as appropriate
  • Collaborate with other staff on the transition of participants into permanent supportive housing
  • team members. 
  • Assures positive member outcomes by providing exceptional member care and timely documentation of services by consistently meeting optimal engagement and performance standards. 
  • Adherence to all agency policies, licensure, and training requirements.
  • To ensure compliance and adequate services, additional job duties may be required to meet the needs of the program and/or department.

MINIMUM QUALIFICATIONS

  • Bachelor’s degree in Human Services, Social Work, Psychology, or a related field and a minimum of one (1) year of experience working with individuals experiencing homelessness and/or with mental health needs;
  • Or an equivalent combination of education and directly related experience acceptable.
  • Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire TB test clearance required upon hire and annually thereafter.
  • Must pass a pre-employment drug screening.
  • Valid driver’s license with acceptable driving record; ability to transport participants in company or personal vehicle.

 

KNOWLEDGE, SKILLS, ABILITIES:

  • Demonstrated ability to work effectively with diverse populations experiencing homelessness, behavioral health challenges, and complex needs.
  • Strong interpersonal and communication skills, with the ability to build trust and work effectively with diverse populations.
  • Demonstrated professionalism, reliability, and integrity in all interactions.
  • Ability to problem-solve, adapt to changing priorities, and manage a caseload in a fast-paced environment.
  • Skilled in crisis intervention, de-escalation, and trauma-informed care practices.
  • Proficiency in computer systems, including Microsoft Office, EHR/EMR platforms, and HMIS data entry.
  • Ability to prioritize tasks, maintain confidentiality, and adhere to HIPAA regulations.
  • Team-oriented mindset with the ability to work independently when needed.
  • Ability to visually monitor environments and respond appropriately in crisis situations.
