Plans, develops and implements all development strategies that support Hospice of the Western Reserve’s mission and long-term goals. Oversees all activities of Hospice of the Western Reserve Foundation. Manages a development portfolio and leads the development team in fostering a dynamic and comprehensive funding program. 

 

WHAT YOU WILL DO: 

• Responsible for assisting with routine visits, skilled procedures, IV needs and dressing change visits 

• Responsible for symptom control visits, admission visits, Plan of Care visits and/or discharge visits as assigned 

• To assist with initiation and coordination of continuous care in collaboration with Team Leader 

• Responsible to do death visits and/or emergency visits per site and arrange for admissions/respites as needed 

• Maintain and promptly complete all necessary documentation and reports related to hospice care according to agency standards 

• Assists with covering primary RN’s caseload during vacations or leaves of absence 

• Assists with covering primary RN sick calls by taking calls, visits if needed 

• Responsible to check On-Call schedule and back up support list and will inform staff of their assignment 

• To participate in the On-Call rotation 

• To assist primary RN when time is needed for charting and attending meetings 

• Responsible for assisting teams with ordering medication and supplies 

• To assist other teams at other sites as needed and requested by Team Leaders 

• Responsible to assist Team Leader with other duties as needed and assigned 

• To practice in accordance with Hospice of the Western Reserve clinical standards of practice, policies and procedures 

• Assess physical, psychosocial and spiritual needs of the patient and family/caregivers 

• Provide intermittent skilled nursing services to the patient under the patient’s physician’s directions 

• In collaboration with team, monitor and evaluate pain and symptoms and provide information and assistance to the patient and family to manage those problems 

• Serve as liaison to the patient’s physician(s) when needed; provide information to the physician based on knowledge related to the patient’s needs 

• Provide emotional support to the patient and family/caregiver 

• Refer patients and/or family members to other services when appropriate and follow up as necessary 

• Provide instruction (health teaching) to the patient and family 

• Participate in care conferences and bereavement follow-up 

• Participate in case review, quality improvement activities and other meetings as assigned 

• Plan and clinically supervise activities of HNA as related to patient care with assigned patients 

• Communicate by phone or in writing to case managers as determined case by case 

• Assist team with coordination of the Plan of Care which includes all disciplines including volunteer staff 

• Participate in coordinating activities of the volunteer staff as they relate to specific patient/family needs 

• Demonstrate positive customer service responsiveness according to agency standards 

• Flexible in meeting patient/family needs that require variable hours 

• Maintain Transmission Based Precautions 

• Participate in coordination of and communications with volunteer staff 

• Complete annual competency requirements 

• Other duties as assigned 

• Participate in public relations, community outreach and promotion of Hospice of the Western Reserve 

• Assist with orientation of new employees 

• Assist with in-service programs as needed 

• Serve on committees as appropriate or assigned 

• Assist with education/precepting of students 

 

SUCCESS CAPABILITIES:

• Licensed RN in the State of Ohio 

• BSN preferred 

• Experience preferred in pain and symptom management; oncology; public health; or strong medical/surgical background with Knowledge/Skills/Abilities 

• Must have and maintain a valid driver’s license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR’s insurer (not a requirement for Hospice House staff)

CONDITIONS OF EMPLOYMENT:

  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
  • Provide an active driver’s license
  • Ability to provide proof of eligibility to work in the United States.
  • High level of integrity, ethics and professionalism.

 

Physical 

This is medium to heavy work requiring the exertion of up to 100 pounds of force occasionally and up to 20 pounds of force frequently, and the use of appropriate safety transfer techniques and body mechanics when moving, transporting, etc. of patients, sometimes in excess of 150 pounds; work requires stooping, reaching, standing, walking, lifting, fingering, grasping, and feeling; vocal communication; hearing; visual acuity and color perception. Employee understand and accepts possibility of exposure to environmental elements, internal and external, such as infectious disease, blood-borne pathogens, and chemicals and/or chemical fumes, and adverse effects of inclement weather that may occur.
