Pay Range: $52,800-$97,500 + Flexible Schedule
Extended Foster Care Program Overview
Ohana's Extended Foster Care (EFC) program is in direct partnership with the Arizona Department of Child Safety that serves young adults (ages 17.5 to 21) who are aging out of the foster care system. An EFC Success Coach is a unique role that combines a focus on helping young adults achieve their goals (education, employment, housing, health, relationships, etc.), as well as providing the necessary case management that equips them for a successful transition to adulthood.
EFC Success Coach & Case Manager Description
Carry a caseload of 20 young adults who are aging out of foster care system
Complete a minimum of 35 monthly in-person engagements in order to build positive relationships and help the young adult achieve their goals
Document all progress towards achieving case goals utilizing the DCS database

Minimum Requirements
Shall possess a bachelor’s degree or equivalent skills acquired through experience
Shall be at least 26 years of age
Valid AZ driver's license & car insurance
Clear Background Check and a valid fingerprint clearance card

Salary ranges from $52,800-97,500 based on education and experience
