The Community Health Worker (CHW) supports Enhanced Care Management (ECM) by engaging Medi-Cal members, addressing health-related social needs, coordinating services, and helping members navigate clinical and community resources. Working as a trusted liaison, the CHW conducts outreach, builds rapport, supports care plan activities, and facilitates “warm handoffs” to medical, behavioral health, social services, and Community Supports providers. The CHW also assists the ECM program with screening, assessment, and post-enrollment services in coordination with other team members. This role advances health equity by delivering culturally responsive, strengths-based support in community and home settings, in alignment with CalAIM and Managed Care Plan requirements.
The pay rate for this role is $26.50 per hour.
ESSENTIAL FUNCTIONS
- The CHW conducts community and home-based outreach to identify and engage eligible ECM members referred by plans, providers, or community partners, while building trusting relationships through trauma-informed, culturally responsive approaches and addressing barriers such as transportation, language, and digital access
- The CHW assists with intake, screening tools, and social needs assessments to gather member-identified goals and priorities, and collaborates with the interdisciplinary team (RN/PN, Behavioral Health Coordinator, Care Coordinator) to implement and update individualized care plans.
- Arrange and accompany members to appointments as appropriate; provide appointment reminders and follow-ups.
- Facilitate referrals and “warm handoffs” to clinical services, behavioral health, social services, housing/community supports, and recovery resources.
- Help members apply for benefits (e.g., CalFresh), obtain IDs, and connect to transportation, housing navigation, and community programs.
- Provide strengths-based coaching and health education (e.g., chronic disease self-management basics, medication pick-ups, follow-up care).
- Use motivational interviewing and harm-reduction principles to promote self-efficacy and adherence to care goals.
- Document activities, encounters, referrals, and outcomes in the Electronic Health Record (EHR)/care management platform in a timely, accurate manner.
- Track service delivery per ECM and Community Supports guidelines; support data collection for quality improvement.
- Participate in team huddles, case conferencing, and trainings; communicate member progress, barriers, and resource gaps.
- Maintain professional boundaries and confidentiality per HIPAA and program policies.
- Maintain current knowledge of local resources; develop relationships with clinics, CBOs, and social service agencies to improve continuity of care.
JOB QUALIFICATIONS
Education/Experience
- Experience in outreach, navigation, or case management support (healthcare, social services, behavioral health, housing, or similar).
- Familiarity with CalAIM/ECM goals, or willingness to learn.
- Strong communication skills, empathy, and ability to work with diverse populations (multilingual/bicultural preferred).
- Proficiency with basic computer tools (email, Word/Excel or Google Workspace) and comfort using EHR/care platforms (training provided).
- Reliable transportation for field work and valid CA driver’s license (or alternative means to meet job duties)
Certificates/Licenses/Clearances
- Training or experience with motivational interviewing, harm-reduction, and strengths-based case management. Knowledge of Inland Empire community resources (health, behavioral health, housing, food security, benefits, transportation).
- Prior CHW certification or completion of a CHW training program
- Bilingual in English and Farsi/Pashto preferred.
Schedule
- Part-time (~20 hrs/week). Some early evening or weekend hours may be required for member availability and outreach events; schedules set with supervisor.

- Proficient skill set in using an array of Microsoft Office Suite software programs such as Word, Excel, PowerPoint, Access, Adobe Reader, One Note, Outlook, Publisher, Teams, Outlook, Zoom etc.
- Able to multi-task and set workload priorities for time sensitive projects/tasks.
- Ability to problem solve and make recommendations to processes, policies, etc.
- Able to communicate with all levels of personnel, e.g., written, verbal, in a professional and concise/clear manner; ability to work within a project team and/or independently.
- Able to work in a very diverse environment and with diverse individuals.
- Ability to be flexible in meeting changing work tasks and timelines; must be dependable and reliable.
