PrimeCare Medical is currently looking for an Alcohol and Substance Abuse Case Manager to work in the medical department at the Chester County Prison in West Chester, PA.
This is a full time Monday- Friday position, 40 hours per week, during normal business hours. (8:00am-4:30pm).
Job Responsibilities:
The Case Manager is responsible for assisting in the coordination and implementation of the behavioral health program.
Other duties include:
- Responsible for the monitoring, coordination, and implementation of the MAT (Medication Assisted Treatment) program in accordance with NCCHC/ACA/MCCS Standards.
- Assist in the coordination of the admission and placement of inmates/patients in alcohol and substance abuse facilities. Initiate the paperwork required for the inmates/patient’s admission.
- Document services provided in the electronic medical record in a complete, accurate and timely manner.
- Monitor inmates/patients to interventions and report any concerns to supervisory staff.
- Assist patients in developing a discharge plan to continue MAT (Medication Assisted Treatment) services in the community.
- Assist inmate/patient with proper medical assistance application.
- Coordinate with community providers to assist inmates/patients in identifying resources that are available to them post-release.
- Obtain releases from patients to request and obtain records from patient's previous treatment providers.
- Serve as a member of various committees within the medical unit and facility, as necessary.
- Participate in all in-service education programs according to the annual in-service calendar.
- Maintain confidentiality at ALL times.
- Any/all other duties and responsibilities as assigned or delegated.

Requirements
- Bachelor's Degree in Social Work, Psychology, Counseling, or other related Human Services degrees with a concentration in mental health course work preferred.
- A minimum of one (1) year clinical experience and/or experience in the Drug and Alcohol field is preferred.
