Job Title: Tribal Family Services Prevention Case Manager

Reports To: Tribal Family Services Director

FLSA Status: Non-Exempt

Classification: Regular, Full-Time, 40 hours/week

Schedule: Monday-Friday, 8:00am-4:30pm (must be flexible for home visitations)

Location: Onsite (Valley Center and surrounding area)

Funding: Grant ends June 30, 2027

 

SUMMARY:

The Tribal Family Services Prevention Case Manager (TFS Prevention Case Manager) assists Indian families by providing in-home parenting and child abuse prevention services. The TFS Prevention Case Manager will work closely with TFS Court Advocate and San Diego County Health and Human Services Agency (HHSA) to provide information, resources, and referrals to prevent the breakup of Indian families.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES: The information in this job description indicates the general nature and level of work performed. 1. Work closely with HHSA and other community agencies to prevent child abuse through education, referrals, and assessments in San Diego and Riverside Counties.

2. Work with county welfare services on investigations of abuse and neglect referrals for seven (7) of IHC’s consortium tribes.

3. Provide case management services to families at-risk and/or involved in prevention/voluntary cases with child welfare services.

4. Conduct community presentations on child abuse prevention and provide parent education classes to families at-risk and/or involved in the child welfare process.

5. Work closely with all departments in the clinic, interacting with providers in medical, dental, behavioral health, and other staff to coordinate services for clients.

6. Attend and contribute to the ICWA case staffing and Child Protective Team (CPT) meetings to support client’s needs, address community issues, and promote resources.

7. Willingness to cross train in other aspects of Tribal Family Services.

8. Provide statistical information as required by policy and procedures.

9. Compliant with all program and grant reporting.

10. Position may be re-assigned during activation of EOP.

 

SUPERVISORY RESPONSIBILITIES

This position has no supervisory responsibilities.

 

TRAVEL & DRIVING REQUIREMENTS

· Travel may be required for this position.

· Driving is required for this position.

 

EDUCATION AND/OR EXPERIENCE

· High school diploma or GED required.

· Bachelor’s degree in social work or related field preferred.

· Minimum of one (1) year related experience and the willingness to be trained.

 

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

· Knowledge of ICWA regulations, County CPS agency.

· Knowledge of HIPAA and Child Protection laws.

· Knowledge of culturally diverse populations and the willingness to be trained.

· Skilled in parent education, community presentations and facilitating meetings.

· Skilled in interviewing, community presentations, facilitating meetings.

· Skilled in obtaining information and cooperation from individuals or groups.

· Ability to establish and maintain effective working relationships with patients, staff, and the public.

· Ability to interpret and communicate social work agency rules, regulations, and procedures.

· Ability to write reports, correctly reflect on treatment and document patient progress.

· Productivity Software: Microsoft Office 365; Word, Excel, PowerPoint, Email & Calendar Management (outlook), Teams (video conferencing and meetings), and Share Point.

· Electronic health records software (EPIC MyChart preferred).

· Utilization of Paycom and PolicyTech systems.

 

CERTIFICATES, LICENSES, MEMBERSHIPS AND REGULATIONS

· A valid California driver’s license and a good driving record will be required at the time of appointment and must be maintained throughout employment.

· Current AED CPR certification or attainment within 6 months of hire.

· Applicants must successfully pass a pre-employment physical exam, tuberculin skin test or x-ray and urine drug screen test.

· Health must be adequate to perform all duties of the position.

· Must pass criminal background check.

 

WORK ENVIRONMENT

· Indoor office environment. Occasional work conducted off-site in home (home visits and supervision).
