About the opportunity

The Clinical Care Coordinator is supported by the Care Manager and Service Manager and works in a multidisciplinary team, providing clinical leadership to nursing and care staff. They work collaboratively with all levels of management, clinicians, carers, residents, families, support staff and relevant community agencies to facilitate an effective resident focused service.

You will:

  • Provide quality person-focused care to residents and support care and nursing staff to provide exemplary care in a home like environment.

  • Ensure clinical care and nursing assessment is of a high calibre and that care is delivered in line with the Aged Care Standards, Resident Choice, and the policy and procedures of the organisation.

  • Work closely with the Care Manager to ensure that national quality indicators are reported and below benchmark to and assist to drive clinical improvement in the home for our residents.

To thrive in this role, you will have:

  • Excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve.

  • A compassionate, flexible and positive approach to effective
    person-centred care.

  • The ability to nurture friendly, respectful relationships with your team, residents and families and contribute to a collaborative team environment with staff and visiting health professionals.

  • Previous aged care experience in a supervisory role.

You will also be required to provide evidence of, or in the process of obtaining:

  • A current National Disability Insurance Scheme (NDIS) workers check OR evidence of application for NDIS check.

  • Proof of current influenza vaccination and other vaccination status consistent with public health orders.
