JWCH Mission Statement:
The Mission of JWCH Institute is to improve the health status and well-being of under-served segments of the population of Los Angeles Area through the direct provision or coordination of health care, health education, services, and research.
Recuperative Care (RCC) Program:
MLK Recuperative Care Center is an Interim Housing program that assists homeless, medically compromised individuals to recover and improve their health conditions. These individuals may also suffer from mental health and substance abuse issues, sometimes causing behaviors such as yelling, cussing, and outbursts of anger.  The role of each staff at MLK RCC is to assist the client in overcoming barriers and increasing coping skills to decrease negative behaviors.
Position Purpose:

The Interim Housing Registered Nurse will participate as a care team member of the MLK Recuperative Care Center (RCC), which provides intensive case management services, behavioral healthcare and offers a satellite medical clinic, on-site. This is funded by DHS, in the Housing for Health program. JWCH Institute, a Federally Qualified Health Center, is the largest non-government provider of homeless health and supportive services in Los Angeles County. The MLK RCC is a 96 bed program that provides primary care, intensive case management and supportive service to clients who are chronically homeless and have chronic medical conditions and behavioral health diagnoses.

Principal Responsibilities:

  1. Provides individualized and specialized nursing care to a specific client population (homeless).
  2. Participates in all case conferences regarding MLK RCC clients, supporting care team's in treatment plans
  3. Conducts nursing assessments and intakes for new admissions.
  4. Follow established policies and procedures within scope of practice.
  5. Completes TB screenings/readings and documentation, as needed.
  6. Administers oral, IM, SC, IV and SL medications as needed, during clinical hours.
  7. Assists the Medical Provider with procedures as necessary
  8. Complete corrective action plans, if any, post internal/external audits.
  9. Monitors medication management process to assist clients/patients in medication self-administration.
  10. Provides guidance and support to MLK RCC staff, regarding medical care, as needed.
  11. Assists in discharge planning needs.
  12. Coordinates delivery of care with other services and disciplines to achieve-optimal outcomes.
  13. Identify barriers to housing and establish goals and treatment plans.
  14. Takes patient complaints. Follows appropriate procedures in resolving any complaint.
  15. Provides patient/client education and teaching. Schedules patient hospital or doctors (specialist) appointment, if necessary.
  16. Organizes and maintains medication inventory; to include removing expired medications and packaging them for proper disposal, in coordination with LVNs.
  17. Preparation and dispensing of all medications as ordered by the Provider; to include the completion of dispensed medication labels.
  18. Orders medical supplies as needed.
  19. Collaborates with the Program Director and assists with the continuing improvement of systematic functions.
  20. Develops and monitors the communication network between the multi-disciplinary MLK RCC staff and external professionals to improve continuity of care and to avoid conflicting information conveyed to clients/patients regarding their treatment plan.
  21. Ensure all PHI information is maintained in accordance with HIPAA regulations.
  22. Support program leadership in delivering all required services and meets outcome measures established by the Department of Health Services.
  23. Ensure quality measures, regulatory standards and customer service standards are met.

  25. As needed, assist Program Director with reports on operational issues, organizational opportunities and/or programmatic goals.
  26. Monitors and maintains adherence to the Nursing Training Manual and Agency Policies and Procedures.
  27. Performs other duties as required by the program and/or clinic.

Requirements:

  1. Licensed by the State of California to practice as a Registered Nurse.
  2. CPR on an annual basis. Training may be provided.
  3. At least one-year experience in an ambulatory care setting.
  4. Experience working with patients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
  5. Experience working with chronically homeless individuals
  6. Knowledge of and experience with wound care needs.
  7. Knowledge of and experience with populations who have mental health diagnoses.
  8. Knowledge of and experience with populations who have substance abuse.
  9. Bilingual (English/Spanish) preferred.

All shifts Monday thru Friday: 7:00am-7:30pm and 7:00pm-7:30am

Condition of per-diem hire must be willing to work the following schedules:
Fridays: 7:00am-7:30pm and 7:00pm-7:30am
Saturdays: 7:00am-7:30pm and 7:00pm-7:30am
Sundays: 7:00am-7:30pm and 7:00pm-7:30am
