RN /LPN Intake | Hybrid | Full time
Atrio Home Care, a partnership between Holland Home and Resthaven, is looking for a dedicated Nurse to join our intake team, providing essential administrative support in the Grand Rapids and Lakeshore areas. This hybrid role requires a minimum of two days onsite, allowing for a balance of in-person and remote work.
Key Responsibilities:
- Receive client referrals and assess their appropriateness for admission to Atrio Home Health services, verifying the need for skilled care.
- Confirm insurance coverage and accurately enter data into our systems.
- Collaborate with referral sources to ensure that clients’ care needs are met during their initial home visit with Atrio staff.
- Monitor the status of current clients who have been transferred to hospitals or facilities, completing necessary documentation
- Discuss Atrio Home Health’s philosophy, policies, procedures, services, and programs, while educating clients about home health care as needed.
- Communicate with physicians, discharge planners, and other personnel to ensure continuity of care and a safe transition plan.
- Respond promptly to potential referrals, adjusting schedules as necessary to maintain a high level of customer service.
- Actively work to increase referrals to Atrio Home Health.
Location: Hybrid role combining onsite and remote work supporting Grand Rapids and the Lakeshore.
Schedule/Shift:
- Monday to Friday; Day shift from 8am to 5pm, with flexibility to help you maintain a healthy work-life balance.
- Limited administrative on-call rotation (phone triage only) once every 6-8 weeks.
Compensation:
- Salary; based on years of experience
Qualifications and Requirements:
- Current and valid Michigan RN or LPN license
- CPR certification required
- 1 year minimum of home health experience
- Exceptional attention to detail and ability to produce high quality documentation
- Strong computer skills, specifically in Excel and Microsoft
