POSITION SUMMARY
Conducts comprehensive behavioral health assessments on youth and adults seeking mental health, substance use or dual disorder treatment in person or via tele-health. Ensures clients are matched with the appropriate level of care and resources to increase successful treatment and recovery.
CORE RESPONSIBILITIES
• Sets the tone for the treatment experience, establishing clear expectations and building trust to foster engagement in services.
• Conducts thorough intake interviews and assessments in person or via tele-health.
• Gathers psychosocial history, completes mental status examinations, administer symptom-screening tools and render provisional DSM-5 Diagnoses.
• Conducts structured assessments of social determinants of health (housing, employment, transportation, food security etc.)
• Facilitates client assignment to an appropriate long-term treatment provider within the Center to ensure continuity of care.
• Synthesizes findings to develop clear treatment recommendations and individualized care plans.
• Works closely with internal providers and external partners to connect clients with appropriate services.
• Provides clear, compassionate communication to clients and families regarding next steps in care.
• Participates in interdisciplinary team to ensure continuity and quality of care.
• Maintain concurrent, timely and clinically sound records in the electronic health record (EHR).
• Compile, input and maintain accurate data for state-required reporting systems, including CCAR and DACOD, ensuring compliance with regulatory and funding requirements.
• Adhere to HIPPA and confidentiality regulations.
• Participate in clinical supervision, team meetings and ongoing professional development opportunities.
• Contribute to quality improvement initiatives within the department.
• Support a culture of collaboration, inclusion and continuous learning.
• Perform additional responsibilities as assigned in alignment with agency needs and client care.
Requirements
EDUCATION
Master’s Degree in Counseling, Psychology, Social Work, or another Human Services Field
CERTIFICATES, LICENSES, REGISTRATIONS
Requires a license in the State of Colorado as an LPC, LMFT or LCSW
SKILLS, KNOWLEDGE, AND ABILITIES

• Ability to skillfully perform clinical activities in a virtual environment.
• Familiar with the DSM and diagnostic techniques.
• Effective time management skills and the ability to multi-task.
• Ability to work as a team member.
• Computer literacy (email, word-processing, basic computing applications, video platforms, etc.).
• Ability to work flexible hours.
• Bi-lingual abilities will be compensated at an enhanced rate.
