Where you'll be working:
Population and Community Health (PaCH) is part of South Eastern Sydney Local Health District.
PaCH provides community health services that give care to individuals, families and carers, as well as population health services that focus on the whole community or key groups within the community.
PaCH's services are provided at the District's hospitals, community-based facilities including child and family health centres, community health centres and specialist clinics. We work with other service providers across South Eastern Sydney Local Health District, including hospitals and mental health services.

The Role:

Provide a high level specialised/technical support for the management of the National Disability Insurance Scheme Implementation and Disability Inclusion Action Plan to support the efficient and effective operation of the Disability Strategy Unit to achieve business and service objectives. This position is primarily responsible for providing support to the Disability Strategy Manager to develop processes, pathways and training required to assist SESLHD staff to support people with a disability (PWD), their families and carers in their journey through the health care system to accessing appropriate disability care. This position is also responsible for facilitating clinical and systemic escalations and identifying and developing appropriate pathways for PWD with complex needs to access the NDIS or alternate supports.
- Relevant tertiary qualifications in Health or relevant equivalent work experience, or a combination of study and work experience
- Demonstrated high level analytical and problem solving skills including, the ability to provide authoritative advice and recommendations across a large and complex organisation.
- Demonstrated high level negotiation and facilitation skills and commitment to customer service.
- Effective time management and demonstrated ability to prioritise and meet conflicting deadlines.
- Highly developed communication, interpersonal and influencing skills.
- Ability to develop and maintain effective working relationships with senior management, and other key stakeholders.
- Demonstrated experience in planning and evaluation at strategic and service levels.
- Current drivers licence (with a willingness to travel in accordance with the demands of the position).
