Position Summary:
Reporting to the Manager, Patient Flow, the Flow Facilitator supports admitted patients by collaborating with inter-disciplinary teams to identify and expedite barriers to discharge and support optimization of available inpatient capacity
- Uses clinical knowledge to evaluate new and admitted patients to support appropriate patient placement, transfer and discharge
- Collaborates with physicians and clinical teams to proactively identify and address process gaps that may hinder discharge within 72 hours or impede a patient’s safe return to the community setting
- Monitors progress of patients approaching discharge readiness
- Encourages throughput and expedites outstanding diagnostic tests/ treatments within a standard timeframe to advance patients’ same day/next day discharge plans
- Escalates delays to Unit leadership when necessary to support timely transitions of care
- Evaluates opportunities to create capacity (e.g./ co-horting opportunities, utilization of over census beds)
- Collaborates with the Patient Flow Coordinator (PFC) to support implementation of site-specific flow plan
- Attends bed meetings for situational awareness, risk and mitigation strategies
- Uses a collaborative approach to maintain effective communication with key stakeholders
- Provides guidance and direction to CQCL/CN/CL, Professional staff and care teams.
- Utilizes flow and capacity management tools (including Epic) to support flow standard work
- Participates in patient flow improvement work through identification of improvement opportunities, and participation as a subject matter expert.
Qualifications:
- Undergraduate degree (or equivalent) in nursing
- Registered Nurse with the College of Nurses of Ontario
- Minimum 5 years recent clinical experience in an acute care hospital setting
- Proven leadership skills
- Self-directed and thrives in a dynamic environment
- Familiarity with Epic software, preferred
- Provides patient centred care
- Demonstrates positive, collaborative working relationship with internal stakeholders
- Excellent written and verbal communication skills
- Excellent organizational and time management skills, demonstrating attention to detail
- Experience in acute medicine and/or discharge planning is an asset
- Patient Flow experience an asset
- Positive work attendance is essential
