CORE RESPONSIBILITIES:
Personal care is a requirement
- Assisting clients with hygiene needs (bathing, grooming, toileting)
- Supporting with dressing
- Meal preparation and feeding assistance
- Medication reminders and health-related support
- Supports individuals with maintaining a safe household. Adhering to and proving individualized support outlined in the Individual Support Plan.
- Accepts and works within established household routines.
- Accepts assigned responsibilities for laundry, shopping, meal planning and preparation, including household cleaning.
- CFL has a no-single lift policy, Direct Support Professionals are fully trained and capable in appropriate movement procedures for the people we support, including usage of hoyer lifts.
- Supports with hygiene and daily health needs, including but not limited to, shower assistance, personal care, cleaning and medication.
- Adheres to filling out Medication Administration Form (MAR) & signing of chore lists.
- Welcomes and respects the individual’s friends and other visitors.
- Assists the individual in developing and maintaining positive relationships in the community.
- Maintains appropriate time keeping reflecting accurate hours worked.
- Communicates effectively to appropriate CFL management in the event of accidents as required by Incident Reporting procedures.
- Attends routine house meetings, workshops and training as requested.
- Provide reliable transportation to and from appointments, grocery store, community activities and events.
- Accesses CFL email on a weekly basis; checking emails, utilizes to communicate necessary information to CFL management.
- Accesses CFL scheduling platform on a weekly basis to ensure accuracy of services provided. Direct Support Professionals are responsible for ensuring their schedules are accurate and notifying appropriate CFL management of discrepancies.

Physical Demands
- While performing the responsibilities of the job, the employee is required to talk and hear. The employee may be required to assist a person in movement from bed to chair or chair to vehicle. The employee may be required to assist a person from chair to shower or bed via hoyer lift. The employee is often required to sit and use their hands and fingers, to handle or feel. The employee is required to stand, walk, reach with arms and hands, climb or balance, and to stoop, kneel, crouch or crawl. Vision abilities required by this job include close vision.
Education: High School Diploma preferred.
- Experience: A minimum of six months of working in the field of human services preferred.
Special skills and knowledge:
Must possess an understanding of and commitment to the philosophy of integration, normalization, and individual-driven services. Direct Support Professionals must be willing to carry out and share responsibilities, consistently be on time for their shifts, and respect confidential information. Direct Support Professionals provide support in the areas of personal care support (e.g., ability to assist with physical transfers of individuals) and household management. Direct Support Professionals possess the ability to work well with others in a team environment. Individuals are required to be current in basic first aid and CPR certification within 90 days of employment. Proof of current certification must be provided every two years at renewal. Hours of employment are based on the individual support needs and may change unexpectedly.
