Case Manager
Manages a caseload of Family Self-Sufficiency Program clients by conducting eligibility screenings, developing individualized service plans, coordinating resources and group activities, maintaining accurate records, and collaborating with internal and external partners to support clients' progress toward employment and self-sufficiency.
Job Responsibilities

Responsible for screening, enrolling and evaluating application eligibility for Family Self Sufficiency Program clients.

Conducts primary case management of assigned caseload of 50-60 clients by preparing accurate and timely individualized service plans for each adult participant which includes specific and measurable short- and long-term goals and objectives. Monitors client through monthly office visits with all clients.

Schedules and coordinates group activities and presents at monthly meetings for clients.

Maintains up to date and accurate client files and databases as required by agency programmatic policie3s and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies

Knowledgeable about community resources and links clients to appropriate resources. Advocates on behalf of clients to ensure access to services for which the client is eligible.

Coordinates client services with the program's Employment Specialist to help ensure the client can meet short- and long-term goals related to employment.

Solicits, logs, sorts, transports and places in storage in kind donations for the program. Delivers needed basic household items and other resources to client's home.

Participates in community meetings and represents the agency regarding services provided as well as providing accurate and timely reporting to the department director of the meeting discussions.

Education and Experience Requirements:
Relevant Education:
Minimum: Bachelors Degree
Preferred: Masters Degree
Relevant Experience:
Minimum: 1-2 Years
Preferred: 2-4 Years
Must have strong communication, organizational and problem solving skills.
Must possess a general knowledge of community resources.
