Description of Duties: The individual in this role will serve as Team Lead of the Intensive Case Management Team. The focus of this program is to provide community rehabilitative services that provide direct treatment and restorative interventions as well as case management in a community and home setting. The goal of the ICM Team Lead is to lead the team in providing high quality care to the most severe and persistent mentally ill individuals in the community.   The lead will provide direct service at a minimum direct contact hours of 14 hours per week and provide direct supervision for the intensive case management team to include case managers assigned to the team in the service area.  The lead must effectively manage team to assist the client served in reducing the presenting psychiatric or substance abuse symptoms and promoting symptom stability, restoring the recipient’s community living and interpersonal skills, providing first responder intervention to deescalate crisis episodes and ensure linkage to community services and resources. Responsibilities of the ICM Lead include:  Provides individual therapy and family interventions for clients served by the team.  Provides evidenced based interventions including Solution Focused, Cognitive Behavioral Therapy.  Designates the appropriate team staff so that specialized clinical expertise is applied as clinically indicated for each client.  Provides and coordinates the assessment and reassessment of the client’s clinical needs.  Provides clinical expertise and guidance to the ICM team members in applying interventions with the client.  Provides the clinical supervision of all members of the team for the provision of the service.  Determines team caseload by the level of acuity and the needs of the individual served.   Facilitates weekly team meetings of the ICM Team.  Facilitates monthly staff meetings to address changes in caseload, policy updates, high risk individuals and needs in providing person centered ongoing clinical care etc.  Monitors each individual’s physical health, clinical status and response to treatment.   On call 24/7 and will keep the on-call cell phone on at all times. Able to perform 1013/2013.  Attend weekly Interdisciplinary team meetings to collaborate on going clinical care of the individual served.  Ensure referral management and follow up when indicated. Transporting individuals served is a requirement.

Minimum Qualifications: Active LAPC, LAMFT, LMSW Licensure OR Licensed by Georgia State Composite Board to practice independently as a Licensed Professional Counselor (LPC) Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT) OR Registered Nurse (RN) with experience in Psychiatric Nursing. Must be willing to work a 24/7 day a week program. Must have a valid, unrestricted GA Driver’s License.  Must be computer proficient.

Preferred Qualifications: Two years post licensure experience, documented experience working with adults with SPMI.   Two years of supervising experience and management in a behavioral healthcare setting.  At least 6 months of experience working with individuals in community and home setting.  Strong communication skills to work with community stakeholders.  In depth comprehension of health care policy, administration and systemic implications.  Knowledge of DBHDD role and functions in statewide healthcare delivery system.  Adept public speaking capabilities with experience in community education and system coordination.  Approved Clinical Supervisor/Certified Clinical Supervisor Credential.
