The Harris Center for Mental Health and IDD (“The HARRIS CENTER”) is looking for a Service Coordinator to join our team. This person is responsible for providing assistance to individuals with intellectual disabilities in gaining access to services and supports utilizing a person directed planning process to achieve their desired outcomes. Services and supports may include waiver programs, medical, psychiatric, residential, financial, and a wide array of social services in the community. Provides service monitoring through contacts with consumers, Legally Authorized Representatives, program providers and collaterals. The ideal candidate will display compassion, education, proficiency and experience while assisting the patient. This critical role assists in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient.
Your role in action:
- Complete all assessments as specified per department policy and procedural guidelines.
- Based on consumer needs, monitor client at a minimum of a quarterly basis
- Update consumer files as needed
- All documentation is completed within established timelines.
- Complies with all safety, health, and training requirements.
What qualifications you will need:
Education:
- Bachelor’s degree (B.A.) from four-year accredited college or university with a major in a social, behavioral, or human service field, including but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice.
Experience:
- Minimum of one year experience in the human services field.
- Experience working with persons with intellectual disabilities preferred.
Other Key Qualifications:
- Must maintain a valid Texas Driver's License and be insurable under Agency policy.
Knowledge Skills & Abilities:
- Experience working with persons with intellectual disabilities preferred.
- Must possess effective written, verbal communication, and organizational skills.
- Must be able to work autonomously in a fast-paced environment.
- Effectively communicates and works with consumers, family members, visitors and fellow employees in a manner that demonstrates knowledge of cultural diversities.

- Field Based / Reports to Southwest location (9401 Southwest Freeway, Houston, TX 77074) as needed.
- Position requires extensive travel to consumer homes, flexible scheduling and on call status. May be exposed to unfavorable conditions. Interacts with individuals who may be emotional or physically hostile, agitated or uncooperative.
- The position serves as a critical bridge between consumers, legally authorized representative, program providers and other key collaborators. The
Service Coordinator position requires conducting comprehensive assessments to ensure departmental policies and procedural state and agency guidelines are followed. SCs create and review all person directed service plans (initial/annual/revision), conduct monthly face-to-face monitoring visits, follow-up, and provided written documentation of the individuals progression of their goals. They assist with potential threats toward an individual and managing crisis/issues the individuals, providers, and families may have. The SC monitors the overall health and safety of the individuals
