At AMFM Healthcare, we are committed to providing transformative mental health care with compassion, integrity, and clinical excellence. We’re currently seeking a dedicated Clinical Liaison to join our team and help ensure a seamless client experience from admission through aftercare.
About the Role:
AMFM Healthcare is seeking a dedicated and highly organized Clinical Liaison to serve as a vital link between our clinical team, admissions department, outreach representatives, and referral sources. This role ensures seamless communication and coordination of client care, both during treatment and through aftercare planning. The ideal candidate is a proactive professional with strong relationship-building skills and a passion for supporting individuals on their recovery journey.
Key Responsibilities
Referral & Care Coordination
Serve as the primary point of contact for all referent updates, including interventionists, educational consultants, and care coordinators.
Work closely with the admissions and clinical teams to coordinate aftercare planning and higher levels of care (HLOC) step-ups as needed.
Establish and maintain strong relationships with referral sources and community partners.
Assist the clinical team with ACA blocking and client support.
Discharge & Aftercare Planning
Ensure all clients have a comprehensive discharge plan with scheduled appointments for continued care upon leaving AMFM Healthcare.
Connect clients to AMFM's alumni programs before discharge, ensuring each client cohort attends a meet-and-greet session with the Alumni Director.
Provide updates and support to families, collaborating with them to develop and execute effective aftercare plans.
Collaboration & Outreach
Participate in clinical staffing meetings to provide insights and updates on client progress.
Work closely with the referral relations and outreach teams to enhance program visibility and client success. Responsible for scheduling in-services for partner programs to present to our clinical teams regularly.
Attend program tours, networking events, and community meetings to strengthen professional relationships and promote AMFM Healthcare’s services.
Gather quality reviews on Google and Yelp by engaging with satisfied clients and families, encouraging them to share their experiences to help others seeking care.
Customer Satisfaction
Actively engage with clients and families during treatment to ensure a high-quality experience and address any emerging concerns promptly and empathetically.
Collaborate with internal teams to resolve client issues, providing timely follow-up and support to ensure satisfaction.
Proactively gather positive reviews from clients and families who have had a beneficial experience, guiding them through the process of leaving feedback on platforms such as Google and Yelp.
Monitor feedback and sentiment to identify trends, celebrate successes, and implement service improvements where needed.
What We’re Looking For:
Minimum Requirements:
1+ years of experience in behavioral health, case management, or clinical coordination
Possess valid driver’s license with ability to be insured by our auto insurance policy which includes, but not limited to, the following requirements: 4 continuous years of driving experience, 21 years of age or older, less than 3 moving violations in the last 3 years, and no more than 1 major violation in the last 3 years.
Successful completion of the ADP Selective Screening Services background check as well as any other state, federal or company required background/record check.
Skills & Knowledge:
- Knowledge of mental health and substance use treatment programs, aftercare resources, and alumni support systems.
- Strong interpersonal and communication skills to collaborate effectively with internal teams, clients, families, and external partners.
- Knowledge of the clinical, medical, and miscellaneous services provided to the clients.
- Ability to multitask, prioritize, and manage time efficiently in a fast-paced environment.
- Excel at both written and verbal communications, able to send emails and communicate in a professional, appropriate tone to convey essential information; must be able to utilize the proper linguistic syntax and spelling.
- Ability to operate standard office machines and equipment, including telephones, computers, copy machines, fax machines, calculators, scanners, and shredders.
- Proficient multi-tasking capabilities with excellent communication skills and a positive attitude
- Knowledge of computer skills including Microsoft Office, Adobe Acrobat, G Suite products, and SalesForce CRM.
Physical Requirements:
Ability to sit for long periods and use standard office equipment
Able to lift up to 10 lbs
Must be comfortable using electronic systems and participating in virtual communications
