ABOUT THE ORGANIZATION:

Operation PAR, Inc. is a non-profit organization offering the full continuum of services for individuals with substance use and/or mental health disorders. PAR has been providing comprehensive behavioral health treatment in west Florida since 1970 through advocacy, education, clinical services, and research.

GENERAL DESCRIPTION: Provides case management services related to mental health and chemical dependency for patients enrolled in Medication Assisted Treatment program (MAPS), including those engaged through the mobile MAT unit and while incarcerated. Services include outreach, identification, screening, assessment, and referral to community resources to address their health, social, and economic needs. Utilizes time spent in the mobile MAT unit and county jails to assist with initiating Medication Assisted Treatment (MAT) services, providing evaluation, screening, and assessment to support engagement in treatment and recovery support. Case managers play a key role in care coordination for incarcerated individuals, including assisting with the transition from jail-based MAT services to outpatient MAT and other community-based treatment programs. Develops individualized transition plans to ensure post-incarceration continuity of care and access to recovery support services, such as peer coaching, housing, employment, and other wraparound supports, to reduce recidivism and support long-term recovery. Extensive liaison work with referral sources is required. Responsible for tracking assigned clients and maintaining appropriate records in a timely and accurate manner. Also responsible for monitoring and expanding referral networks with agencies and organizations.   Must have a reliable vehicle to be able to visit local referral sources, jails, and mobile MAT unit locations.

 

Scope of Practice:     Clinical Services provided by the Case Manager are limited to the following:

  • Screening
  • Psychosocial assessment
  • Treatment planning
  • Referral
  • Service coordination and case management
  • Consultation
  • Continuing assessment and treatment plan reviews
  • Recovery support services
  • Crisis intervention
  • Individual, family, and community education
  • Documentation of progress

REQUIRED QUALIFICATIONS: Must meet at least one of the following criteria:

  1. Bachelor’s or master’s degree from an accredited university or college with a major in counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field.
  2. Registered marriage and family therapy, clinical social work, and mental health counseling intern.
  3. Certified master’s degree level addiction professional (MCAP) certified by the Florida Certification Board
  4. Certified addictions professional (CAP) certified by the Florida Certification Board
  5. Certified addiction counselor (CAC) certified by the Florida Certification Board
  6. Certified recovery peer specialist (CRPS) or certified recovery support specialist (CRSS) certified by the Florida Certification Board.

 

LICENSES, CERTIFICATIONS AND REGISTRATIONS:

  • Valid Florida driver's license
  • CPR and First Aid certification to be completed within six months of employment

ESSENTIAL PHYSICAL/TECHNICAL SKILLS:

  • Ability to write accurately and legibly
  • Moderate proficiency in the use of Microsoft Windows-based computers
  • Ability to accurately type at least 40 words per minute
  • Ability to sit at a desk and view a display screen for extended periods of time
  • Ability to bend, stoop, and reach
  • Ability to sit in meetings for extended periods of time
  • Ability to move throughout program facilities both indoors and outdoors
  • Ability to lift and carry up to 10 pounds
  • Good hearing and vision
  • Ability to drive a mobile treatment unit


ENVIRONMENTAL CONDITIONS:

  • Treatment program / Office / Mobile treatment facility / County jail environment
  • Fast-paced environment
  • Exposure to possible verbal abuse and similar behavior
  • Possible exposure to patients/clients with communicable diseases
  • Willingness to work in multiple counties as part of a mobile MAT unit
