Job Location:    TCN Xenia - Xenia, OH
Salary Range:    $18.00 - $22.00 Hourly

Description

You'll be responsible for intensive case management services as identified by the ACT (Assertive Community Treatment) clinical team for individuals diagnosed with moderate to severe mental illness and co-occurring disorders.

 

Primary Responsibilities:

  • Provide individual case management under clinical supervision, meeting productivity and quality requirements set by the Agency and within the limits of services permitted by position and scope of practice.
  • Develop and maintain quality, accurate clinical charts for all clients served.
  • Serve as a liaison between the hospital, criminal justice system, and other treatment services in the community.
  • Adheres to standards pertaining to assessments and documentation.
  • Work collaboratively with the multidisciplinary ACT team, including psychiatrists, nurses, social workers, and vocational specialists, to deliver comprehensive care.
  • Establish and maintain relationships with community agencies, service providers, and other stakeholders.

Work Location and Standard Work Hours:

  • Xenia, OH
  • Full-time
  • Some evenings and weekends may be required
  • Shifts may vary.

Qualifications


  • Education: bachelor's degree in human/social service field, or associate degree with equivalent field experience.
  • Experience: 3 years working with SMD population
  • Certification:  First Aid; CPR; De-escalation
  • Other: Valid Ohio driver’s license; proof of automobile liability insurance; meets OMHAS licensure and certification requirements; competencies for serving the SMD population.
