Position Summary:

The Community Health Outreach Supervisor will focus on building relationships, educating the community, promoting health programs and guiding patients on insurance and non-insurance options.  The supervisor will develop and implement outreach strategies, connect with community organizations and healthcare providers.

 

Responsibilities: 

  • Schedule, performance manage, mentor and train direct reports in alignment to company goals, mission, vision and core values. 
  • Partner with recruitment to staff TAPM’s outreach team.
  • Manage and coordinate activities of TAPM’s outreach programs including in-reach events, health screenings, and other requests for organizations, businesses, and municipalities to attract patients and delegate tasks to direct reports
  • Oversee program coordination, marketing, communications, program delivery, tracking and metrics.
  • Produce local reports on programs performance and provide feedback to management.
  • Obtain and negotiate contractual agreements with local resources in partnership with COO, CFBDO and CEO
  • Assists in provider-patient care relationship building. 
  • Participate and maintain required trainings to engage the patient population.
  • Represents the organization at local community meetings including but not limited to: NC Works Partners Meeting, Guilford Community Care Network, Housing coalitions, Partners Ending Homelessness, etc. 
  • Foster and maintain relationships with community organizations, healthcare providers and other stakeholders as needed.
  • Oversee outreach events, timelines, deliverables and ensuring outreach events are operated effectively.
  • Collaborate with quality improvement department for monthly quality initiative focus areas to assist with making patients aware.
  • Collaborate with practice administrators and sites to encourage patients to participate in my chart for easier patient access and care.
  • Collaborate and Coordinate in reach events with PHP’s(primary health plans) to provide our patients with information regarding their plans.
  • The ability to complete and process TAPM sliding fee scale applications and train others as needed.

Gain full understanding of GCCN eligibility application and train the team accordingly.
