We are growing and seeking to add a full-time Intake RN/Home Care Coordinator to our fabulous team of health care professionals here at Care Resources!

 

As the Intake RN/Home Care Coordinator, you will:

·         Be responsible for coordinating home care services for participants

·         Make initial visits and subsequent home safety assessments

·         Complete ongoing evaluations of participant needs, supplies, education, and ongoing quality of care

·         Provide nursing visits in the home for wound care, foley management, disease education, etc.

·         Schedule: Monday – Friday day shift position

o   includes participation in the on-call rotation: must be available one weekend every 6 weeks, 8am – 5pm to make visits if needed

 

Qualified Candidates will have:

·         The job requires licensure as an RN in the state of Michigan.

·         Prior experience working with a frail or elderly population

·         Prior home care experience 

·         The ability to multi-task, work independently as well as in a team setting, and have a passion for those whom we serve through this amazing program

ABOUT CARE RESOURCES:

Care Resources operates the PACE program (Program of All-inclusive Care for the Elderly) which is located in our wonderful facility at 4150 Kalamazoo Avenue SE in Grand Rapids.  Care Resources provides and coordinates all medical care and social services to eligible participants. We provide all physician and nursing care, prescription drugs, durable medical equipment, dental, eye and foot care, hospital care, medical transportation, home care, emergency, rehab, social and recreational activities at the Day Center. In order to be eligible for the PACE program, seniors must be 55 and older, a Kent County resident, able to live safely at home and meet the nursing facility level of care.
