JOB SUMMARY: The Specialist I will work directly with the Lead Guardian to support the role of
court appointed guardians within the program. Types of support will involve coordinating and
attending medical appointments, arranging for insurance coverage and other services, running client
errands such as shopping, delivering of items, client check-in visits and other tasks. May also include
support of conservator clients and coordination of tasks. Clients consist primarily of the elderly
population with advanced forms of cognitive impairment (Dementia, Alzheimer's, etc.).

 

ESSENTIAL JOB RESPONSIBILITIES:

  • Visits clients for monthly check-in.
  • Attends routine medical, dental, or other appointments as an advocate and reports on activity.
  • Assist the lead guardian in tasks that involve driving, shopping, delivering items, sorting personal items for donation, or protecting, visiting client for routine check-ins, obtaining medical information.
  • Assist with the coordination of appointments and services for the client(s).
  • Communicate with care facility staff, medical professions, and other service providers to coordinate the best care for the protected person.
  • Understands and follows best practices for guardians and conservators, always making decisions in the best interest of the Protected Person.
  • Provides accurate documentation and record-keeping relating to all services provided to clients in accordance with the agency’s quality assurance standards.
  • Assist in support of conservator and other client types if needed.

 

Reports to: Senior GAP Program Manager

Qualifications


REQUIREMENTS AND QUALIFICATIONS:

 

  • Experience working with elderly/senior population.
  • Ability to multi-task and problem-solve complex situations.
  • Must have your own car and be able to drive for client and program-related tasks (includes mileage reimbursement).
  • Case management type of experience including overseeing and coordination of medical care.
  • Willingness to obtain certification through CGC as Professional Fiduciary, within one year.
  • Excellent oral and written communication skills.
  • Working knowledge of basic computer programs, including Microsoft programs and g-mail.
  • Good organizational skills and ability to prioritize a variety of tasks that can change daily.
  • Ability and willingness to work as an agency team member and support the mission, values and goals of Impact NW.
  • Must have reliable transportation to visit clients and other client-related work.
  • Presents in a professional manner when engaged in business on behalf of the Program.
  • Sensitivity and ability to relate to people from diverse lifestyles, socioeconomic levels, and cultures
  • Completion of required training within the agency.
  • A willingness to learn other roles within the Program as time allows and needs arise.

 

PHYSICAL ACTIVITIES:

  • Repetitive motion, prolonged sitting, and extensive visual involvement.
  • Ability to efficiently travel to perform duties and other agency related errands.
  • Moderate lifting; up to 20 pounds.
