Community Based Palliative Care Manager (Nurse Practitioner)


Schedule

  • Full-time, salaried leadership role

    • Your typical schedule will be Monday to Friday 8 AM to 5 PM

  • Rotating on-call duties

  • Flexibility required for occasional evening/weekend needs


What You’ll Be Doing

As our Palliative Care Program Manager (Nurse Practitioner), you’ll take the lead on the day-to-day operations of our community-based palliative care program. You'll be the go-to person for staff support, patient care coordination, and quality improvement efforts—all while making sure we’re delivering compassionate, high-quality care that aligns with our mission.

This role combines leadership, clinical expertise, strategic thinking, and people skills. You’ll manage a dedicated team, collaborate with other departments, and help shape the future of our palliative care services.

Here’s what your days might include:

  • Running the show: Oversee daily operations of the palliative care program—including staffing, scheduling, and patient care activities

  • Supporting the team: Hire, onboard, coach, and develop your clinical team to ensure high performance and professional growth

  • Championing quality: Lead initiatives that improve patient/family outcomes, resource use, and program performance

  • Collaborating with access and intake: Ensure smooth patient admissions and referrals in partnership with our Patient Access Center

  • Guiding the care plan: Lead IDT (interdisciplinary team) meetings, utilization reviews, and care coordination with outside partners

  • Keeping an eye on the numbers: Track budgets, recommend improvements, and monitor variances

  • Engaging the community: Act as a liaison to hospitals, facilities, and healthcare professionals; offer education on palliative care and billing best practices

  • Planning for the future: Participate in strategic planning and help grow the program thoughtfully and sustainably

  • Exploring growth: Identify and support grant opportunities and alternative funding for program development

  • Staying current: Maintain your own clinical and professional knowledge—and encourage your team to do the same


What We’re Looking For

We’re searching for a nurse practitioner who’s passionate about palliative care and ready to lead with compassion, confidence, and a collaborative spirit.

You’ll be a great fit if you have:

  • A master’s degree in nursing and certification/licensure as a nurse practitioner (North Carolina)

  • At least 5 years of experience in a palliative care setting

  • Experience leading clinical teams and navigating complex healthcare systems

  • A deep understanding of regulatory standards, documentation, and quality measures

  • A strong nursing foundation and a knack for creative, people-centered problem-solving

  • Excellent communication and presentation skills—written and verbal

  • A commitment to continuous learning and team development

  • A valid driver’s license and reliable transportation

  • Comfort working with all age groups, from infants to older adults

  • The ability to flex and adapt: evenings, weekends, and on-call coverage are part of the mix

Bonus points if you’re certified in hospice or palliative care (CHPN, ACHPN).
