🎯 What You’ll Do in This Role:
The Nurse Practitioner (NP) provides high-quality, comprehensive medical care and healthcare services to participants of Huron Valley PACE. As a key member of the interdisciplinary care team, the NP is responsible for assessing, diagnosing, and managing participants’ health needs in collaboration with other clinical professionals.
Primary Responsibilities:
Patient Care & Clinical Responsibilities:
- Conduct comprehensive health histories, physical examinations, and patient education.
- Order, interpret, and follow up on laboratory and diagnostic test results.
- Perform selected medical procedures and ensure timely, accurate documentation of care.
- Assess, initiate, and coordinate daily patient care in collaboration with the attending physician.
- Identify and respond to abnormalities requiring urgent intervention or specialist referral.
- Initiate and manage orders for medications, diagnostic studies, and treatments under physician-approved protocols.
- Provide emergency care and initiate appropriate emergency measures as needed.
- Coordinate care transitions with emergency department physicians and hospital staff to reduce avoidable hospitalizations and ensure timely discharges.
Collaboration & Care Coordination:
- Serve as a liaison among physicians, nurses, and interdisciplinary team members to ensure cohesive, participant-centered care.
- Consult with attending physicians to initiate referrals to contracted specialists and services as needed.
- Coordinate care across various settings, including hospitals, skilled nursing facilities, assisted living, and adult foster care homes.
- Actively participate in Interdisciplinary Team (IDT) meetings and contribute to the development and execution of holistic care plans.
Patient & Family Education:
- Educate participants and their families about disease prevention, chronic condition management, and health maintenance.
- Engage in family conferences to share updates, support decision-making, and guide care planning.
Quality Improvement & Compliance:
- Participate in quality improvement (QI) initiatives aimed at enhancing patient outcomes and care delivery processes.
- Represent Huron Valley PACE in meetings with regulatory agencies, professional organizations, and community partners.
Documentation & Reporting:
- Accurately document changes in participant status, interventions, and outcomes in the electronic medical record.
- Communicate significant clinical updates to team members to support timely and coordinated care delivery.
✅ What You’ll Need:
- Bachelor’s degree in nursing from an accredited college or university.
- Completion of an accredited Nurse Practitioner program.
- Master’s degree in clinical nursing preferred.
- Current Registered Nurse (RN) license in the State of Michigan.
- Certification as a Nurse Practitioner by an accredited certifying body (e.g., ANCC or AANP).
- Proof of active licensure and certification required at time of hire.
- 5–7 years of total nursing experience.
- Minimum of 3 years practicing as a Nurse Practitioner, or master’s degree in clinical nursing with at least 5 years of nursing experience.
- At least 1 year of primary care experience with frail, elderly, or long-term care populations.
Desired Qualifications:
- Experience working in a PACE (Program of All-Inclusive Care for the Elderly) setting.
- Experience in a managed care environment.
- 2–3 years of direct nursing experience with geriatric patients.
