Position Summary:
Lakes Region VNA and Hospice is seeking a compassionate and skilled Home Care and Hospice Social Worker to join our interdisciplinary home health team. In this role, you will provide personalized emotional and social support to patients in their homes, helping them navigate complex medical and life situations with dignity and care.
Key Responsibilities:
- Conduct comprehensive psychosocial assessments considering social, economic, cultural, and behavioral factors.
- Provide crisis intervention, client advocacy, and referrals.
- Collaborate with healthcare teams and community resources to develop individualized care plans.
- Maintain confidentiality and uphold the NASW Code of Ethics.
- Support patients with complex medical and psychosocial needs.
- Navigate reimbursement systems and advocate for continued care coverage.
- Document all interventions accurately and in a timely manner.
- Stay informed on federal, state, and local assistance programs and community resources.
Qualifications:
- Master’s degree in social work preferred, BSW with supervision will be considered.
- Strong interpersonal skills and ability to handle sensitive situations with tact and diplomacy.
- Proficiency in Microsoft Office applications.
- Excellent communication, problem-solving, and time management skills.
- Ability to multitask in a dynamic environment.
