Position Title: Support Coordinator – Quality
Department: Case Management (CMA)
Reports To: CMA Quality Supervisor
FLSA Status: Non-Exempt

Position Summary:

The Quality Support Coordinator plays a vital role in promoting the health, safety, and rights of individuals served by TRE by supporting the review, tracking, and analysis of incidents and documentation across case management operations. This position assists in reviewing Incident Reports (IRs) and Critical Incidents (CIRs), collaborates with external entities such as Adult/Child Protective Services (APS/CPS) and law enforcement for investigations of Mistreatment, and supports risk mitigation efforts. The Quality Support Coordinator also assists with Human Rights Committee (HRC) facilitation and reviews documentation to identify gaps in services, inconsistencies, or systemic issues that require follow-up.

 

Key Responsibilities:

Quality Monitoring and Compliance:

  • Review incident reports (IRs and CIRs) for individuals served to ensure accurate classification, timely submission, and compliance with applicable regulations.
  • Identify trends, systemic issues, or gaps in services through documentation reviews and data analysis.
  • Partner with external agencies (APS, CPS, law enforcement) during investigations into allegations of Mistreatment.
  • Support the coordination and facilitation of Human Rights Committee (HRC) meetings, including documentation review and follow-up.
  • Review internal and external documentation for alignment with individual needs, risk areas, and service requirements.

Documentation and Systems Management:

  • Maintain accurate and timely notes, reports, and documentation using multiple internal and external systems.
  • Track and report critical data points for regulatory submissions and internal quality improvement efforts.
  • Assist in compiling documentation and materials to support quality audits and reviews by governing entities (HCPF, CDPHE, etc.).

Team and Operational Support:

  • Provide internal support to the Case Management team by ensuring compliance with critical incident timelines and quality standards.
  • Collaborate with coordinators and supervisors to enhance service planning, risk mitigation, and continuity of care.
  • Participate in internal trainings, team meetings, and professional development opportunities to remain up to date with policies and expectations.
  • Provide coverage and assistance to other CMA team roles as needed.

Customer Service and Communication:

  • Communicate professionally and respectfully with individuals served, families, providers, and partner agencies.
  • Uphold TRE’s values of empathy, inclusion, and person-centered support in all written and verbal communications.
  • Maintain HIPAA and confidentiality standards throughout all interactions and documentation.

 

Qualifications:

Minimum:

  • Bachelor’s degree in a human behavioral science or related field.
  • In lieu of a degree, a combination of relevant education and 5 years of verifiable experience working with Long-Term Services and Supports (LTSS) populations in a public or private agency.
  • Strong written communication and interpersonal skills.
  • Ability to manage multiple priorities and meet required timelines.
  • Proficiency in Microsoft 365 (Outlook, Teams, Word, Excel, SharePoint) and ability to learn internal and State systems.
  • Understanding of HCPF, CDPHE, and CMS guidelines related to LTSS and incident reporting.

Preferred:

  • Experience working in case management or quality assurance roles.
  • Familiarity with Medicaid waivers and Colorado-specific HCBS service delivery.
  • Knowledge of incident review standards, Human Rights Committees, or Mistreatment protocols.
  • Cultural responsiveness and ability to work with diverse populations.

Tools and Equipment Used:

Computers/tablets, printers, telephones, Microsoft Office Suite, proprietary systems/databases, and personal vehicles.

Working Environment/Physical Activities:

Work is performed in a variety of settings including office, home, virtual, and community locations. Frequent sitting, standing, walking, and use of computers. Occasional lifting up to 20 lbs. A valid Colorado driver’s license (unless exempt due to military status), reliable personal transportation, and minimum insurance coverage of $100,000/$300,000/$100,000 are required.
