Job summary
The primary role of the Assessor is to provide front door and comprehensive intake assessments, per the Montgomery County Coordinated Entry tool, to homeless clients entering the Gateway Shelters for Montgomery County.
Summary of Essential Job Functions
The Assessor will be responsible for:
-completing an average of 4 assessments a day.
-applying criteria for county housing options to reach accurate scores for appropriate housing placements.
-contributing to quality assurance outcomes including time to assessment from shelter entry and contributing to reducing the overall stay in shelter.
-completing high quality clinical documentation at the time of services, as part of Collaborative Documentation efforts.
-submitting reports/spreadsheets weekly.
-meeting productivity goals.
-complete all documentation following agency policies and procedures and utilizing HMIS appropriately.
-participating in ongoing education and training to develop professional skills, achieve QMHS status via Homefull training, and maintain licensure (if applicable).
-complete CPST documentation and billing spreadsheets following guidelines.
-maintain high standard of client care, contributing to an environment where all clients are treated with dignity and respect.
-completing other duties as assigned.
Minimum Requirements
Bachelor’s degree in social work, counseling, psychology, or related field required. Experience/knowledge of homelessness and/or marginalized communities preferred.
Abilities Required
Computer skills including Word, Excel, and Power Point.
