Mission Connection offers a comprehensive level of mental healthcare for individuals who need more than traditional, once-a-week therapy. Our flexible Partial Hospitalization Program (PHP) supports clients through structured, evidence-based treatment—delivered by compassionate clinicians committed to lasting impact. We’re looking for a Clinical Coordinator to help ensure a smooth, supportive, and successful client experience from intake through discharge.
Mission Connection extends AMFM Healthcare’s compassion-driven, evidence-based clinical program. Our innovative hybrid model seamlessly blends in-person and telehealth services, ensuring you can access the care you need, wherever you are. We provide mental health support to those struggling with primary mental health diagnoses such as anxiety, depression, and mood disorders. Mission Connection empowers these individuals to connect with their clinical team when, where, and how it’s most convenient for them.
About the Role:
As Clinical Coordinator, you’ll work closely with the Clinical Director and play a hands-on role in managing day-to-day clinical programming and client care. You’ll be a central figure in maintaining clinical quality and consistency, while helping shape a safe and connected healing environment.
Key Responsibilities:
Ensure clients are engaging appropriately and providing support where needed to ensure clients are meeting requirements.
Maintain the record of care in all clients Electronic Health Record (EHR) in accordance with company policy, Joint Commission accreditation requirements, and specific state guidelines.
Complete and support the intake and administrative documentation for the client census and see that related documents are accurate and consistently up to date.
Arrange and assist in new admissions, specifically scheduling and/or completing intakes and initial assessments for clients.
Monitor clients closely that are heightened suicide risks and support with other crisis situations as directed by Clinical Director.
Facilitate clinical services such as, group services and or individual services as directed by the Clinical Director.
Support and complete case management needs and documentation such as exploring barriers, medical/clinical documentation, benefit coordination, etc.
Coordinate continued care in relation to outpatient appointments, housing, and other related after care needs.
Participate in enhancing and formulating program rules and regulations in coordination with the Clinical Director.
Support and facilitate coordination between other departments and stakeholders as needed. Collaborate and support the clinical director and leadership regarding organizational growth.
What We’re Looking For:
Required Qualifications:
Master’s degree in Psychology, Counseling, Social Work, or a related field
Active registration or licensure (e.g., MFT, LPC, LCSW, PsyD/PhD) in Virginia or eligible for registration
2+ years of experience working with individuals with primary mental health diagnoses
Familiarity with clinical leadership, administrative processes, and documentation
Skills & Abilities:
Strong verbal and written communication skills
Team-oriented mindset with a positive, professional demeanor
Familiarity with EHR systems, Microsoft Office, and Google Workspace
Understanding of clinical and medical terminology relevant to behavioral healthcare
Ability to respond calmly and effectively to crisis situations
