Role Description
At Oak Street, success isn’t completing the visit or hitting an R.V.U. target. Success is high quality care: (1) appropriate hospital admissions, (2) excellent preventive care/chronic disease management, and (3) a great patient experience. In other words: success at Oak Street is helping our patients be happy, healthy, and home. @Home Medical Directors embrace these goals tightly, envision the means to achieve them, and lead the @Home providers in their market (region) in pursuit of these ends. They set an example of clinical excellence and supervise and coach our @Home providers. @Home Medical Directors also drive operational initiatives/continuous improvement, liaise with colleagues in brick-and-mortar Oak Street clinics, and generally ensure the program operates smoothly.
Overview of Responsibilities
Set an example of clinical excellence
Build dyad relationship with the regional vice president
Support other practice managers and partner on all program priorities
Supervise and coach medical directors
Define and achieve key performance indicators for operational and clinical excellence
Sharing date and helping medical directors understand and use it
Supporting accurate coding and documentation
Owning market performance reviews
Implement a culture of clinical excellence, professional development and support, and camaraderie in line with Oak Street Values
Supervise and coach providers (Nurse Practitioners and Behavioral Health Specialist), including:
Coaching on data-driven practices to keep patients happy, healthy, and out of the hospital
Sharing data and helping providers understand it
Ensuring accurate coding and documentation
Owning provider performance reviews
Build relationships with team
Assist @Home Program Manager with supporting/coaching other team members (CIS/MAs and MOCs) as needed.
Ensure our patients are always delighted with the level of care they receive
Problem solve patient and team member issues
Lead the execution of continuous improvement initiatives
Advocate for our program, team, and patients
Act as an ambassador in the local social/community organizations
Build relationships with clinic SMD’s, CMDs and other healthcare leadership
What are we looking for?
Ideal candidates will be
M.D., D.O.
Board certification in Internal Medicine or Family Medicine required for M.D. or D.O.,
Experience in clinical leadership roles, leading and coaching providers to be the best they can be for their patients and their colleagues.
Experience using a metrics-driven approach to the provision of medical care and/or quality projects.
Passionate about their work and motivated to be a part of a fast growing organization.
Aware of their leadership skills and potential for impact and eager to share those with a fast-growing, energetic team on the leading edge of healthcare innovation.
Commitment to older adults and to low-income patients.
Comfort with ambiguity and a love for problem solving.
Ability to remain calm in the midst of the storm, guiding others to smart, effective solutions.
5+ years of experience in outpatient practice preferred.
Fellowship training in Geriatrics, other professional degrees (e.g., M.B.A., J.D., M.P.H.), and prior executive roles welcomed but not required.

Pay Range
The typical pay range for this role is:
$184,112.00 - $396,550.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. This position also includes an award target in the company’s equity award program.
