Job Title: Intake Representative – Supportive Care

Position Overview

As an Intake Representative in Supportive Care, you will be responsible for processing referrals for home care, hospice, and rehabilitation services. You will verify patient information, assist with coordination of benefits and care needs, and work directly with patients, families, agencies, and internal teams. This role plays a vital part in ensuring seamless transitions of care and addressing Social Determinants of Health (SDOH) through patient-centered communication and data coordination. No nights, weekends, or holidays.

Key Responsibilities

• Receive and process referrals for Home Health, DME, Hospice, and Rehab services
• Provide support to patients, caregivers, physicians, and referral sources in person or electronically
• Ensure data integrity and accurate entry of patient demographics, orders, and insurance details
• Contact third-party payors for pre-certification and benefit verification
• Maintain tracking systems and document all interactions in the EMR
• Communicate with home health/hospice agencies across TN, MS, AR, MO, and KY
• Maintain and update referral resources and agency contact lists
• Support patients with insurance forms, financial assistance, and accessing the patient portal
• Assist with SDOH and Rehab pre-authorization follow-ups as needed
• Prepare materials for outside agencies and internal coordination
• Perform other duties as assigned

Qualifications

Education & Experience

• High school diploma or equivalent required
• Minimum one (1) year of experience in a healthcare environment required
• Preferred: Experience with insurance verification or accessing coverage information from third-party payors

Skills & Abilities

• Strong customer service skills with internal and external contacts
• Ability to adapt and respond to complex or problematic situations
• High attention to detail and ability to manage multiple tasks and priorities
• Proficient in Microsoft Word and general office equipment
• Strong written and verbal communication skills
• Ability to work independently and exercise sound judgment
