Under the supervision of the Manager of Case Management, the Social Worker Assists in meeting the psychosocial needs of patients and enabling patients to utilize medical care to achieve their optimal level of health. Assesses patients for discharge planning and assists with referrals to appropriate local/state/ community resources. The incumbent will have knowledge of health care systems, and have demonstrated ability to interact, relate to, work with, and support the activities of a diverse workforce.
The incumbent will carry-out her/his duties by adhering to the highest standards of ethical and moral conduct, acts in the best interest of Three Crosses Regional Hospital, and fully supports the mission, vision, and values of Three Crosses Regional Hospital. The incumbent will have knowledge of health care systems, and have demonstrated ability to interact, relate to, work with, and support the activities of a diverse workforce.
Primary Responsibilities
- Assumes a leadership role and collaborates with the interdisciplinary team to manage care, through criteria driven processes, for the appropriate level of care, patient status, discharge needs and resource utilization.
- Assess and evaluate patient needs throughout their hospitalization, including medical, psychological, and social factors.
- Assesses patients’ post discharge needs and facilitates the provision of services necessary to meet identified needs.
- Develop individualized care plans for patients, collaborating with interdisciplinary teams to ensure comprehensive care. Develop discharge plans in collaboration with the patient, family, and healthcare providers to ensure continuity of care after hospital discharge.
- Coordinate home care services, rehabilitation, or other post-hospital care services as necessary.
- Facilitate the transition of patients to lower levels of care when appropriate (e.g., rehabilitation centers, skilled nursing facilities).
- Ensure cost-effective care without compromising patient safety or outcomes.
- Attend staff meetings as needed.
- Other duties as assigned.
Education and Experience
- Required: Bachelor’s degree Social Work (BSW)
- Preferred: Master’s degree in Social Work (MSW)
- Preferred: 2 years’ experience in an acute healthcare setting or case management experience
- Preferred: Experience with electronic and case management documentation systems; willing to train
- Required: Current NM licensure as a LBSW Social Worker, LMSW Social Worker (master level), or LISW (Licensed Independent Social Worker) from the New Mexico Board of Social Work Examiners.
- Required: BLS certification
